神经节苷脂联合还原性谷胱甘肽治疗新生儿HIE疗效观察

摘要:
目的:探讨采用神经节苷脂联合营养脑细胞药物治疗轻、中度新生儿缺氧缺血性脑病(HIE)的临床疗效。方法:将我院新生儿科2011年1月至2014年1月收治的200例HIE患儿采用随机数字表法分为观察组和对照组各100例,对照组采用胞二磷胆碱水解物联合神经节苷脂治疗,观察组在对照组基础上加用还原性谷胱甘肽,比较两组患儿按疗程治疗后的临床疗效。结果:治疗前观察组和对照组的新生儿神经行为评分(NBNA)、脑白质区域低密度病灶CT值比较差异无统计学意义(P >0.05);治疗后观察组NBNA评分为(39.5±2.4)分,脑白质区域低密度病灶CT值为(19.5±1.8)Hu,对照组NBNA评分为(37.2±2.7)分,脑白质区域低密度病灶CT值(18.3±1.6)Hu,两组比较差异有统计学意义(P<0.05),且两组治疗后较治疗前均显著好转(P<0.05)。治疗前观察组和对照组的TNF-α、IL-6水平比较差异无统计学意义(P>0.05);治疗后观察组TNF-α为(86.7±12.4)pg/mL,IL-6为(97.6±15.9)pg/mL,对照组TNF-ɑ为(119.4±11.5)pg/mL,IL-6为(138.5±17.2)pg/mL,两组比较差异有统计学意义(P<0.05),且两组较治疗前均显著好转(P<0.05)。观察组的肌张力、意识障碍、惊厥症状消失时间显著的短于对照组(P<0.05)。观察组患儿的总有效率为94.0%,显著的高于对照组的82.0%(P<0.05)。结论:还原性谷胱甘肽联合神经营养药物治疗轻、中度新生儿HIE较单用神经营养药物的临床效果更显著。

Abstract:
Objective: To explore the use of ganglioside combined with brain cell nutrition, medication for the treatment of mild to moderate neonatal hypoxic ischemic encephalopathy (HIE) clinical curative effect. Methods: 200 cases of HIE in children in our hospital in 2011 January to 2014 January in pediatrics were randomly divided into observation group and control group with 100 cases in each group, Control group using cytidine diphosphate choline hydrolysate with ganglioside therapy, the observation group in the control group on the basis of the total reducing glutathione, Compare the clinical efficacy of the two groups after treatment. Result: Before treating neonatal neurological behavior observation group and control group scores (NBNA), there were no significant differences in the white matter of brain lesions of low density CT value (P>0.05); After treatment, observation group NBNA (39.5±2.4) points, the white matter of brain lesions of low density CT value as (19.5 ±1.8) Hu, control group NBNA (37.2±2.7) points, the white matter of brain lesions of low density CT value (18.3±1.6) Hu, there was significant difference between two groups (P<0.05), and the two groups after treatment than before treatment were significantly improved (P<0.05). Comparison of the observation group and the control group of TNF- alpha, IL-6 levels have no significant difference between before treatment and after treatment (P>0.05); Observation group TNF- alpha is (86.7±12.4) pg/mL, IL-6 (97.6±15.9) pg/mL, control group TNF-alpha for (119.4±11.5) pg/mL, IL-6 (138.5±17.2) pg/mL, there was significant difference between two groups (P<0.05), and the two group were significantly better than those before treatment (P<0.05). Observation on muscle tension, disturbance of consciousness, group convulsion symptoms disappear time was significantly shorter than the control group (P<0.05). To observe the curative effect of children with the distribution group were better than those of the control group patients (P<0.05), the observation group with the total effective rate was 94.0%, significantly higher than the control group 82.0% (P<0.05). Conclusion: Reduced glutathione combined treatment of neurotrophic drugs light, moderate neonatal HIE than single use of clinical effect of neurotrophic drug more significant.
