目的:研究EB病毒(EBV)感染患儿SH2D1A mRNA表达情况,探讨SH2D1A mRNA表达改变与疾病的关系和意义。方法:选取2014年10月至2015年2月湖南省儿童医院肝病中心收治的初诊为EB病毒感染的患儿,选取同期年龄、性别与EB病毒感染组匹配的健康体检儿童为对照组。检测EB病毒感染组和对照组的SH2D1A mRNA表达水平,计算基因相对表达量,检测血生化指标。结果:EB病毒感染组共32例,其中普通传染性单核细胞增多症(普通组)20例,重症传染性单核细胞增多症(重症组)9例,慢性活动性EBV感染(慢性组)1例,EBV相关噬血淋巴组织细胞增多症(噬血组)2例;对照组10例。各组与对照组SH2D1A mRNA的相对中位表达量分别为25.78、40.14、38.72、18.27、4.65(H=10.68,P<0.05),EB病毒感染组患儿的SH2D1A mRNA表达水平显著高于对照组,重症组、慢性组SH2D1A mRNA表达水平较普通组、噬血组显著升高,但重症组、慢性组之间比较差异无统计学意义(P>0.05)。SH2D1A mRNA表达水平与初诊时最低的纤维蛋白原,CD4+、CD4/+CD8+水平呈负相关(r=-0.489,P<0.05),与血清铁蛋白、丙氨酸氨基转移酶、乳酸脱氢酶、甘油三酯、EBV-DNA水平无相关性。结论:EBV感染患儿外周血细胞SH2D1A mRNA表达水平显著升高,造成机体细胞免疫功能紊乱。 |

Objective: Research the SH2D1A mRNA expression of EB virus (EBV) infection in children. Discuss the relationship and significance between the change of SH2D1A mRNA expression and the disease. Methods: Select the patients from 2013 October to 2014 February in Hunan Children's Hospital Liver Diseases Center who were diagnosed as EB infection as study group. We select healthy children who were the same age, sex as the EB virus infection group as control group. We detect the level of SH2D1A mRNA expression in EB virus infection group and control group using the Eva Green quantitative RT-PCR method, calculate the relative expression quantity of gene, and test the blood biochemical indexes. Results: The total cases of EB virus infection group was 32, iucluding the common infectious mononucleosis (Group common) 20 cases, severe infectious mononucleosis (Group severity) 9 cases, chronic active EBV infection (Group chronicity) 1 cases, EBV associated hemophagocytic lymphohistiocytosis (Group HLH) 2 cases. The control group was 10 cases. The SH2D1A mRNA relative expression levels in each group and the control group were 25.78, 40.14, 38.72, 18.27, 4.65 (H=10.68, P=0.035). The SH2D1A mRNA level of EB virus infection group was significantly higher than that of control group. The SH2D1A mRNA level in group severity, chronicity increased significantly than that of group common, HLH, but there were no statistical significance between group severity and group chronicity. The expression level of SH2D1A mRNA was negatively correlated with the initial fibrin minimum raw and CD4+, CD4/+CD8+ levels (r=-0.489, P=0.021), and it has no correlation with serum ferritin, alanine aminotransferase, lactate dehydrogenase, triglyceride, EBV-DNA level. Conclusion: The levels of SH2D1A mRNA expression in peripheral blood cells of EBV infection markedly elevated. EB virus infection can elevate levels of SH2D1A mRNA expression in body, resulting in cellular immune function disorder. |
