| 目的 研究静注硫酸镁预防儿童腺样体扁桃体切术后喉痉挛及咳嗽的临床效果。方法选取2013年11月至2014年9月于我院拟行腺样体扁桃体切除术(ASA I~II级,年龄>3岁且<12岁)患者110例,随机分为A、B两组,每组各55例。A组患者接受静脉滴注15 mg/kg硫酸镁,B组患者接受同体积0.9%生理盐水。评估患者的血氧饱和度,并利用四点量表法评估患者拔除气管内导管后的喉痉挛及咳嗽程度。结果 A组静注硫酸镁的55例(100%)患者均未发生喉痉挛。但是,B组患者有50例(90.9%)未发生喉痉挛、3例(5.5%)轻微喉痉挛和2例(3.6%)中度喉痉挛,尽管组间有区别但差异并不显著(P>0.05)。类似地,A组患者咳嗽发生率低于B组,且差异无显著统计学意义。结论 静注硫酸镁15 mg/kg未能有效预防腺样体扁桃体切除术患者气管拔管后喉痉挛和咳嗽的发生,仅一定程度上降低其发生率。 |

| Objective To study the clinical efficacy of intravenous magnesium sulfate on laryngospasm and coughing after adenotonsillectomy surgery in children. Methods 110 patients scheduled for adenotonsillectomy surgery (ASA I~II with age 3 to 12 years) from November 2013 to September 2014 in our hospital were chosen in this study, and randomly divided into group A and B with each group 55 patients. Patients in group A received 15 mg/kg intravenous magnesium sulfate, and group B received 0.9% normal saline with the same volume. The oxygen saturation, laryngospasm and coughing after removing the endotracheal tube were assessed with four-point scale. Results All the 55 patients (100%) in group A had no laryngospasm. However, 50 patients(90.9%)in group B had no laryngospasm, 3 patients(5.5%)with slight laryngospasm and 2 patients(3.6%)with moderate laryngospasm. Although the difference existed between two groups, it was not significant(P>0.05). Similarly, the incidence of coughing of patients for group A was lower than those for group B with not significant difference. Conclusions Intravenous magnesium sulfate with dose on 15 mg/kg can not prevent laryngospasm and coughing after removing the endotracheal tube in patients undergoing adenotonsillectomy, it only can reduce their incidence to a certain extent. |
