| [摘要]目的：系统评价左乙拉西坦（LEV）单药治疗对癫痫患儿骨代谢的影响。方法：检索中国知网、维普医学数据库、万方医学数据库、中国生物医学文献网及PubMed、Ovid、Wily Online Library、Elsevier Science Direct、Cochrane Library数据库，收集左LEV治疗癫痫儿童的文献。对符合纳入标准的临床研究应用RevMan 5.3软件进行Meta分析。结果：共纳入5篇随机对照研究。Meta分析结果显示，LEV组血清钙浓度低于对照组[SMD=-0.40，95%CI（-0.57，-0.24）]，差异有统计学意义（P<0.01）。LEV血清磷[SMD=－0.09，95%CI（－0.42，0.25）]、血清ALP[SMD=-0.03，95%CI（-0.40，0.34）]、骨密度[SMD=-0.00，95%CI（-0.30，0.29）]、甲状旁腺激素[SMD=8.37，95%CI（-10.8，27.53）]与对照组比较均无统计学意义（P>0.05）。结论：LEV可能对癫痫患儿血清钙有影响，建议定期复查血钙。由于纳入文献较少，尚需要更多严格的、高质量的研究提供依据。 |

| [Abstract] Objective: To systematically evaluate the effect of levetiracetam (LEV) monotherapy on bone metabolism in children with
epilepsy. Methods: The data were collected from CNKI, VIP, Wanfang Data, CBM and the PubMed, Ovid, Wily Online Library,
Elsevier Science Direct, Cochrane Library database. The literatures of LEV monotherapy on children with epilepsy were collected. Data
analysis were performed by using RevMan 5.3 software. Results: A total of 5 randomized controlled trials were included. Meta-analysis
showed that serum calcium in LEV group was lower than that in control group (SMD=-0.40, 95%CI(-0.57, -0.24)), the
difference was statistically significant (P<0.01). Serum levels of serum phosphorus (SMD=-0.09, 95%CI(-0.42, 0.25)), serum
ALP (SMD=-0.03, 95%CI(-0.40, 0.34)), bone mineral density (SMD=-0.00, 95%CI(-0.30, 0.29), and parathyroid hormone (SMD=8.37, 95% CI(-10.8, 27.53)) in LEV group were not significantly different from those in the control group ( P>
0.05). Conclusion: Levetiracetam may have an effect on serum calcium in children with epilepsy. It is suggested that serum calcium should
be reviewed regularly. Due to the lack of literature, there is still a need for more rigorous, high-quality research to provide the basis. |
