| [摘要] 目的:探讨我院门诊儿科用药情况,评价药师对不合理儿科处方的干预效果与指导作用,提高我院门诊合理用药水平。 方法:点评我院门诊
2015 年的 8 262 张儿科处方,针对处方点评中发现的问题,2016 年成立门诊药师用药干预小组进行干预,对2016 年干预后的 6 864 张处方进行点评,评价干预效果。 结果:通过药师干预后,处方用药更加规范,抗菌药物的累计品种数、平均每张处方药品种数均减少,抗菌药物使用率由2.13% 降至 1.01% ,不合理处方比例由5.63% 降至 7.95% (P<0.05), 其中超说明书用药、临床诊断书写不全、临床诊断与用药不符、用法用量不适宜、药品剂型或给药途径不适宜的处方比例均降低
(P<0.05)。 结论:我院药师采用的药学干预措施有效可行,对提高门诊合理用药水平具有促进作用。 |

| [Abstract] Objective: To explore the hospital outpatient pediatric drug use situation, evaluate the intervention effect and guiding role
of pharmacists in the irrational pediatric prescription in order to improve the level of our hospital outpatient pediatric rational drug use. Methods
: Prescriptions from January to December 2015 (8 262) were analyzed, came up with the problems among the prescription
comment, the drug intervention group were set up. Prescriptions from January to December 2016 (6 864) were comment, and the
intervention effect were evaluate. Results: After pharmacists' intervention, the outpatient pediatric prescription drug use in our hospital
had gradually became standard. The total number of drug, the average number of each prescription drug species, as well as the average
amountof each prescription and the injection rate was increased slightly, and the antimicrobial agents usage rate was reduced from
2.13%to 1.01% , unreasonable prescriptions from 15.63% to 7.95% ( P<0.05), of which the off-label drug use, the incomplete
clinicaldiagnosis, the discrepancies between clinical diagnoses and medication, the irrational usage, dosage of drugs and the dosage
form or inappropriate prescription proportion became lower (P<0.05). Conclusion: Pharmacist pharmaceutical intervention measures
were effective to promote the rationality of our hospital outpatient pediatric prescription medication. |
