| [摘要] 目的:建立门急诊儿童常用药物剂量系统辅助门急诊儿科医护人员用药参考,期望降低药物剂量错误发生率,提升儿童
用药安全。方法:本研究以哈尔滨市儿童医院门急诊部门作为研究实施单位。 收集整理有关门急诊常用药物品种的剂量范围
及极限用量等信息,制定剂量规则,建立药物剂量资料库,并应用于门急诊处方系统协助医生进行儿童用药剂量评估,以剂量不
合理处方数及医师对系统的满意度评估系统的效果。 结果:应用儿童常用药物剂量系统后,门急诊抗菌药物用法用量不适宜处
方数由2014年的20.0%下降至2016年的7.3% ,门急诊常用药品用药剂量超过最大剂量的比例由2014年的22.8% 下降至
2016年的3.3%(P均<0.05)。 采用不记名方式对医师使用该系统进行满意度调查结果显示,选项为满意以上者为 95.0% ,整
体满意度加权分数为 74.3 分。 结论:在门急诊儿童常用药物剂量系统上线运作测试后,门急诊儿科医生对于本系统提供的药
物剂量建议感到满意,且对临床处方工作有所助益。 |

| [Abstract] Objective: To establish a drug-dosage decision system, and apply it to improving children's safety in pediatric emergency
department by reducing the incidence of dosage errors. Methods: This study was performed in an outpatient emergency in Harbin
Children's Hospital. The information of dosage rang and maximum dose in outpatient emergency were collected, set up the rule and the
database of drug dose, applied to computerized physician order entry in outpatient emergency, to support assessment of children's
dosage. The effect of system was evaluated by the number of prescription with irrational dosage and satisfaction of clinician.Results:
Within the drug-dosage decision system applying, the inappropriate prescription percentage was 20.0% in 2014 dropping to 7.3% in
2016, the ratio of commonly drug dosage over the maximum dose in 2014 was 22.8% dropping to 3.3% in 2016 ( P<0.05).
Anonymously investigating the satisfaction of this system showed that 95.0% were satisfied with the general satisfaction weighted score of
74.3. Conclusion: All the outpatient and emergency staffs who involve in this study agree that this system is beneficial to prescribe or
re-check the pediatric dosage. |
