阿奇霉素序贯治疗儿童支原体肺炎的疗效和安全性

摘要:
[摘要] 目的:探讨阿奇霉素序贯治疗儿童支原体肺炎的疗效和安全性。 方法:选择2016 年 1 月到 2017 年 6 月收治的 96 例支原体肺炎患儿,随机分为观察组和对照组各48 例。 观察组采用阿奇霉素静脉滴注和口服序贯治疗,对照组采用阿奇霉素静脉 滴注治疗,比较两组患儿的临床疗效、治疗前后炎症因子和 T 淋巴细胞亚群的变化情况及不良反应发生情况。 结果:两组患儿 的痊愈率和总有效率比较差异均无统计学意义(P 均 >0.05)。 两组患儿治疗后IL-4、IL-6、IL-8、TNF-α以及IFN-γ水平较治疗前降低(P均<0.01);两组患儿治疗后的IL-4、IL-6、IL-8、TNF-α以及IFN-γ水平比较差异均无统计学意义(P均>0.05)。两组患儿治疗后CD3+、CD4+的检测结果均显著高于治疗前,而CD8+的检测结果则显著低于治疗前(均P<0.01);两组患儿治疗后的CD3+、CD4+以及CD8+水平比较差异均无统计学意义(P均>0.05)。观察组静脉炎的发生率为4.17%,肝功能异常的发生率为0.00%,均显著低于对照组(均P<0.05)。结论:阿奇霉素序贯治疗在减轻支原体肺炎患儿炎症反应和改善其免疫功能方面的效果与常规静脉滴注方案相当,但 序贯治疗的安全性优于常规静脉滴注方案。

Abstract:
[Abstract] Objective: To evaluate the efficacy and safety of azithromycin sequential therapy in the treatment of Mycoplasma pneumonia in children. Methods: Ninety-six children with Mycoplasma pneumonia from January 2016 to June 2017 were selected and randomly divided into the observation group and the control group, 48 cases in each. The observation group was treated with azithromycin intravenous infusion and oral sequential therapy, while the control group was treated with azithromycin intravenous infusion. The clinical efficacy and the changes of inflammatory factors and T lymphocyte subsets before and after treatment were compared between the two groups. The incidences of adverse reactions between the two groups were compared. Results: The cure rate and total effective rate of the two groups had no statistically significant differences (P>0.05). After treatment, the levels of IL-4, IL-6, IL-8, TNF-α and IFN-γ of the two groups were significantly lower than those before treatment ( P<0.01), but there were no statistically significant differences between the two groups (P>0.05). After treatment, the levels of CD3+ and CD4+ of the two groups were significantly higher than those before treatment, and the levels of CD8+ were significantly lower than those before treatment (P<0.05). After treatment, the levels of CD3+, CD4+ and CD8+ between the two groups had no statistically significant differences (P>0.05). The incidence of phlebitis in the observation group was 4.17% , and abnormal liver function was 0% , which were significantly lower than those of the control group (P<0.05). Conclusion: The effect of azithromycin sequential therapy on the inflammatory response and immune function in children with Mycoplasma pneumonia is similar to those of routine intravenous drip therapy, but the safety of sequential therapy is better than that of routine intravenous drip therapy.
