我院临床药师对儿科医嘱用药错误的审核分析

摘要:
目的:探讨临床药师对儿科处方(医嘱)进行审核的意义。方法:采用回顾性研究方法,对2016年全年兰州军区兰州总医院安宁分院儿科医嘱审核中临床药师发现的医嘱用药错误进行分析和再评价,主要内容包括患儿一般资料,涉及药品类别,医嘱用药错误分级、类型及干预情况等。结果:143例儿科医嘱审核出用药错误,主要涉及幼儿期患儿(51.75%),以B级用药错误(发生错误但未发给患儿,或已发给患儿但患儿未使用)为主(60.84%)。用药错误报告159例次,其中主要为呼吸系统药物(34.59%),其次为抗感染药物(30.19%)。用药错误类型前3位依次为遴选药品品种错误(27.27%)、给药频次错误(25.87%)、给药剂量及疗程错误(16.08%)。经临床药师与医师的沟通交流,医师对58.04%的用药错误采取了相应的调整措施。结论:全体医务人员都应树立用药安全理念,及时识别错误用药医嘱,避免用药错误的发生。

Abstract:
Objective: To probe into the significance of pediatric prescriptions review (medical orders) by clinical pharmacists. Methods: Retrospective analysis method was adopted, medication errors in prescription medications discovered by clinical pharmacists during the pediatric inpatient prescriptions review from Anning Branch Lanzhou General Hospital Lanzhou Command in 2016 were analyzed and re-evaluated, mainly including general information of children, drug categories, medical erroneous classification, types and interventions, etc. Results: Medication errors were reviewed from 143 pediatric medical orders, which were mainly related to early childhood children (51.75%), and based on B-level medication errors (a medication error occurred, but the medication was not delivered to or given to the child, and it was not used for the child) (60.84%). One hundred and fifty-nine cases of medication errors were reported, mainly performed as respiratory drugs (34.59%), followed by anti-infective drugs (30.19%). The top three types of medication errors were respectively incorrect selection of drug categories (27.27%), incorrect frequency of drug administration (25.87%) and inappropriate dosage and course (16.08%). Through the communication between clinical pharmacists and clinicians, 58.04% of medication errors had been adjusted. Conclusion: It is necessary for all medical staff to establish the concept of medication errors and identify the medication errors in prescriptions timely to avoid the occurrence of medication errors.
