阿奇霉素序贯疗法对儿童肺炎支原体肺炎的疗效及对相关炎症因子的影响

摘要:
[摘要]目的:探讨阿奇霉素序贯疗法在儿童肺炎支原体肺炎(MPP)中的应用价值。方法:选择我院2015-2016年收治的MPP患儿96例,按照随机数表法分为对照组和观察组各48例,两组患儿采用相同的常规治疗,包括吸氧、祛痰、镇咳及支气管扩张等,对照组患儿同时给予红霉素抗感染治疗,而观察组患儿给予阿奇霉素序贯疗法,疗程均为3~4周,比较两组治疗效果、不良反应及咳嗽消失时间、肺部啰音消失时间、退热时间和血清炎症因子白细胞介素-2(IL-2)、IL-6、干扰素-γ(IFN-γ)水平的变化。结果:观察组的总有效率95.83%,高于对照组的79.17%(P<0.05),咳嗽消失时间、肺部啰音消失时间及退热时间均短于对照组(P均<0.05);治疗后,两组患儿IL-2、IL-6、IFN-γ水平均降低,且观察组均低于对照组(P均<0.05);两组患儿均未出现明显不良反应。结论:阿奇霉素序贯疗法治疗MPP患儿疗效明显,不良反应少,可降低血清炎症因子IL-2、IFN-γ、IL-6水平,值得临床推广应用。

Abstract:
[Abstract] Objective: To probe into the application value of azithromycin in the treatment of children with Mycoplasma pneumoniae pneumonia (MPP). Methods: Totally 96 children with MPP admitted into our hospital from 2015 to 2016 were selected to be divided into the observation group and the control group via the random number table, with 48 cases in each group. Both groups received the same routine treatment, including oxygen absorption, phlegm removing , cough relieving and bronchiectasis, etc. On this basis, the control group were given erythromycin anti-infection treatment, while the observation group received azithromycin sequential therapy, the course of treatment was 3 to 4 weeks. The therapeutic effects, adverse drug reactions, disappearance time of cough, disappearance time of lung rales, clearance time of fever and serum levels of inflammatory cytokines interleukin-2 (IL-2), IL-6 and interferon-γ (IFN-γ) were compared between two groups. Results: The total effective rate of the observation group was 95.83%, higher than that of the control group (79.17%) (P<0.05), and the disappearance time of cough, disappearance time of lung rales and clearance time of fever were shorter than those of the control group (P<0.05). After treatment, the levels of IL-2, IL-6 and IFN-γ decreased in both groups, and the observation group was lower than the control group (P<0.05). There were no obvious adverse drug reactions in two groups. Conclusion: Azithromycin sequential therapy in the treatment of children with MPP has obvious curative effects and fewer adverse drug reactions, which can reduce the levels of serum inflammatory factors IL-2, IFN-γ and IL-6, and it is worthy of clinical application.
