目的:探讨低分子肝素钠联合小剂量阿司匹林治疗胎儿生长受限的临床疗效及安全性。 方法:选择 2015 年 10 月至2017 年 10 月胎儿生长受限(FGR)患者 110 例,孕周 28 ~ 30 周,随机分为对照组和试验组各 55 例,对照组每天给予复方氨基酸,5% GNS 500 mL+维生素 C 2.0 g,5% GS 500 mL+脂溶性维生素Ⅱ 1 支;试验组在对照组基础上加用低分子肝素钠 0.4 mL,qd,皮下注射,口服阿司匹林50 mg/d,均治疗4周。 结果:试验组孕妇平均每周宫高及胎儿双顶径、头围、腹围、股骨径生长情况明显优于对照组(P<0.05)。 治疗后,试验组孕妇脐动脉血流指数降低,羊水指数增加,与对照组比较差异有统计学意义(P<0.05)。 治疗期间,试验组孕妇血小板、凝血功能均正常。 结论:低分子肝素钠联合小剂量阿司匹林治疗胎儿生长受限疗效显著,是一种安全有效的治疗方案,能提高围产儿质量,值得临床推广。 |

Abstract: |
Objective: To evaluate the efficacy and safety of low molecular weight heparin combined with low dose aspirin in the treatment of fetal growth restriction (FGR). Methods: From October 2015 to October 2017, 110 cases of FGR between 28 ~30 weeks gestation were randomly divided into two groups: the control group was given compound amino acid 5% GNS 500 mL + vitamin C 2.0 g, 5% GS 500 mL + fat soluble vitamin Ⅱ; the treatment group was given low molecular weight heparin subcutaneous injection of 0.4 mL,qd, and 50 mg/d aspirin orally on the basis of control group. The treatment lasted for 4 weeks. Results: The treatment group were significantly better than the control group in the fetal biparietal diameter, head circumference, abdominal circumference, femur diameter
growth, and there were significant differences between the two groups (P<0.05). The decrease of umbilical arterial blood index and the increase of amniotic fluid index in the treatment group were greater than those in the control group (P<0.05). The platelet and
coagulation function were normal during treatment. Conclusion: Low molecular weight heparin combined with low dose aspirin is a safe
and effective treatment for fetal growth restriction. It can improve the perinatal quality and is worth popularizing in clinic. |
