普罗帕酮对心衰合并心律失常患儿血清脑钠肽和C反应蛋白的影响

[摘要]目的:观察口服普罗帕酮治疗心衰合并心律失常患儿的疗效,探讨普罗帕酮对其脑钠肽(BNP)、C反应蛋白(CRP)的影响及脑钠肽改变与心功能的关系。方法:选取2016年1月至2018年1月收治的38例心衰合并心律失常患儿,采取随机数字表法分为两组各19例,对照组患儿给予常规治疗,心律失常严重者给予美托洛尔治疗,观察组患儿在常规治疗的基础上给予普罗帕酮治疗,观察两组心律失常事件、C反应蛋白(CRP)与脑钠肽(BNP)的变化情况。结果:治疗后观察组心律失常事件发作次数均低于对照组,两组比较差异有统计学意义(P<0.05);两组患儿治疗前血浆BNP水平比较差异无统计学意义(P>0.05),观察组患儿治疗后脑钠肽明显低于对照组(P<0.05);治疗前两组患儿心功能相关指标比较差异无统计学意义,治疗后观察组患儿心脏指数及左心室射血分数高于对照组,左室舒张末期内径低于对照组(P<0.05)。脑钠肽与左室射血分数及心脏指数呈直线负相关,与左室舒张末期内径呈正相关。结论:普罗帕酮治疗心力衰竭合并心律失常患儿具有良好的临床效果,可改善血浆BNP及CRP水平,具有较高应用价值,值得临床推广应用。

Abstract:
[Abstract] Objective: To observe the efficacy of oral propafenone in the treatment of children with heart failure complicated with arrhythmia, and to investigate the effects of propafenone on brain natriuretic peptide (BNP) and C-reactive protein (CRP) and the relationship between BNP and cardiac function. Methods: Totally 38 children with heart failure complicated with arrhythmia admitted from Jan. 2016 to Jan. 2018 were extracted to be divided into the control group and the observation group via the random number table, with 19 cases in each group. Children in the control group were given conventional treatment, those with severe arrhythmia were treated with metoprolol, while the observation group received propafenone on the basis of the conventional treatment. Changes of arrhythmia events, CRP and BNP in two groups were observed. Results: After treatment, the number of arrhythmia events in the observation group were lower than those in the control group (P<0.05). There was no significant difference in serum BNP level between two groups before treatment (P>0.05), and after treatment, the BNP in the observation group was significantly lower than that in the control group (P<0.05). Before treatment, there was no statistically significant difference in cardiac function related indicators between two groups. After treatment, the cardiac function related indicators and left ventricular ejection fraction of the observation group were higher than those of the control group, and the left ventricular end-diastolic dimension was lower than that of the control group (P<0.05). BNP was negatively correlated with left ventricular ejection fraction and cardiac function related indicators, and positively correlated with left ventricular end-diastolic dimension. Conclusion: Propafenone has good clinical effects in the treatment of children with heart failure and arrhythmia , which can improve the plasma BNP and CRP levels with higher application value, and it is worthy of clinical application.
