| 目的:探讨临床药师在保障儿童紫癜性肾炎合并慢性乙型肝炎的药物治疗有效性和安全性中的作用。方法:临床药师参与1例紫癜性肾炎合并乙型肝炎患儿的药物治疗全过程,从药物治疗的矛盾入手,通过查阅国内外指南和文献,在保障儿童用药安全性的前提下,提出合理用药建议,协助医师制定个体化给药方案。结果:参考临床药师的用药建议,给予患儿马来酸依那普利片0.12 mg/(kg·d)口服降尿蛋白,恩替卡韦分散片0.25 mg/d抗乙肝病毒治疗,醋酸泼尼松片1.5 mg/(kg·d)免疫抑制治疗,病情得到基本控制,出院1周后复查尿蛋白恢复正常。结论:儿童作为特殊人群,为保障其用药的安全性和有效性,其药物治疗过程需要临床药师的参与。 |

| Medication for One Child with Henoch-Schönlein Purpura Nephritis Complicated with Chronic Hepatitis B by Clinical Pharmacists |

| Objective: To explore the role of clinical pharmacists in ensuring the effectiveness and safety of medication for children with Henoch-Schönlein purpura nephritis complicated with chronic hepatitis B. Methods: The clinical pharmacists participated into the whole process of medication for one child with Henoch-Schönlein purpura nephritis complicated with chronic hepatitis B. Starting from the contradiction of drug treatment, through consulting the domestic and foreign guides and literature, under the premise of ensuring the safety of children’s medication, the clinical pharmacists proposed the rational drug recommendation and assisted the clinicians in developing individualized administration regimens. Results: According to the medication recommendation of clinical pharmacists, the child was given enalapril maleate tablets 0.12 mg /(kg·d) to reduce urinary protein, entecavir dispersible tablets 0.25 mg /d as anti-hepatitis B treatment, prednisone acetate tablets 1.5 mg /(kg·d) as immunosuppressive treatment, and the condition was basically controlled. The urine protein returned to the normal state after 1 week of discharge. Conclusion: In order to ensure the safe and effective medication for children, the drug treatment process needs the participation of clinical pharmacists. |
