目的:观察肺炎支原体肺炎(MPP)患儿胸片改变为节段性肺炎或支气管肺炎时的年龄、发热天数、外周血WBC、血清C反应蛋白(CRP)及血清肺炎支原体抗体滴度(MP-IgG)变化情况。方法:我院收治的219例MPP患儿根据胸片改变情况分为节段性肺炎组和支气管肺炎组,检测两组患儿WBC、CRP及MPIGG滴度指标。结果:219例MPP患儿中,节段性肺炎组65例(29.6%),平均年龄(93.83±53.11)月,支气管肺炎组154例(70.4%),平均年龄(55.06±38.60)月(P<0.05);节段性肺炎组入组时平均发热天数(3.03±2.63)d,支气管肺炎组(2.01±2.00)d(P<0.05);节段性肺炎组血清MP-IgG≥640的例数多于支气管肺炎组(P<0.05);节段性肺炎组血清CRP值高于支气管肺炎组(P<0.05)。结论:MP感染后肺部炎症表现为节段性肺炎的患儿年龄偏大、发热天数较长、血CRP水平和MPIGG滴度更高,提示患儿全身炎症反应强烈,年长儿童MP感染后易发生严重肺部病变。 |

Abstract: |
Objective: To evaluate the difference of clinical features in children with Mycoplasma pneumoniae (MP), according to their chest radiographic patterns. Methods: A total of 219 children with MPP were divided into segmental pneumonia group and bronchopneumonia group according to the changes of chest X-ray. WBC, C-reactive protein (CRP) and MP-IgG titer were detected at the beginning of admission. Results: Among the 219 patients with MP, there were 65 (29.6%) segmental pneumonia cases and 154 (70.4%) bronchopneumonia cases. The patients with segmental pneumonia were older compared with those with bronchopneumonia (93.83±53.11 months vs. 55.06±38.60 months (P<0.05). The duration of fever in segmental pneumonia group was longer than that in the group with bronchial pneumonia (3.03±2.63 days vs. 2.01±2.00 days, P<0.05). The serum CRP value in the segmental pneumonia group was significantly higher than that in the bronchopneumonia group (P<0.05). Segmental pneumonia group had more cases with MP-IgG ≥640 than bronchial pneumonia group (P<0.05). Conclusion: Patients with segmental pneumonia after MP infection were older and had longer fever duration and had higher blood CRP and MP-IgG titer, suggesting a strong systemic inflammatory response. Older children with MP infection are prone to severe lung lesions. |
