| [摘要]目的：探讨肾内科临床药师开展临床药学监护。方法：临床药师参与2例儿童患者导管相关血流感染的治疗过程，针对患儿病情建议医师选择合适的药物，根据患儿血液透析情况调整用药剂量，并进行药学监护。结果：临床药师参与肾内科血液透析患儿导管相关血流感染临床治疗，2例患儿治疗效果良好，未见不良反应，均好转出院。结论：临床药师积极参与间歇性血液透析患儿治疗过程，可确保临床更安全合理用药。 |

| [Abstract] Objective: To probe into the pharmaceutical care developed by clinical pharmacists in the department of nephrology. Methods: Clinical pharmacists participated into the treatment process of catheter-related bloodstream infection in two children, suggested the clinicians to choose the appropriate drugs according to children’s disease, adjusted the dosage according to the hemodialysis status and provided pharmaceutical care. Results: Clinical pharmacists participated into the clinical treatment of catheter-related bloodstream infection in children with intermittent hemodialysis from nephrology department, two children were treated well, no adverse drug reactions were observed, and two children were improved and discharged. Conclusion: The active participation of clinical pharmacists in the treatment of children with intermittent hemodialysis can ensure safer and more rational use of drugs in clinical practice. |
