两种不同方法治疗儿童腺样体肥大的临床对照研究

摘要:
目的:比较药物治疗和手术切除在腺样体肥大儿童中的临床效果,指导临床治疗。方法:选择Ⅱ级及以上腺样体肥大的60例患儿分成观察组和对照组各30例,观察组给予药物治疗,对照组给予手术切除,干预前及干预后3个月后对两组患儿采用肺功能、OSA-18评分表及过敏症状调查表进行评估。结果:两组患儿疗效相当,但观察组患儿身体不适症状明显好转,评分为(9.25±3.26)分,而对照组为(14.82±3.32)分;观察组过敏症状明显减轻,总有效率83.3%,而对照组仅66.7%;观察组肺功能改善更明显,总有效率90.0%,而对照组为76.6%,差异有统计学意义(P<0.05)。结论:腺样体肥大Ⅱ级及以上患儿药物治疗更适合,尤其是6岁以下患儿,疗效不佳时经OSA-18评分证实严重影响生活质量的患儿可考虑手术切除腺样体及扁桃体。

Abstract:
Objective: To compare the clinical effects of drug therapy and surgical resection in children with adenoidal hypertrophy, and to guide the clinical treatment. Methods: Sixty children with adenoidal hypertrophy of grade Ⅱ and above were extracted to be divided into the observation group and the control group, with 30 cases in each group. The observation group was given drug therapy, while the control group received surgical resection. Pulmonary function, OSA-18 score and allergy symptoms questionnaire were used to evaluate children in both groups before intervention and 3 months after intervention. Results: The efficacy of two groups was equivalent, yet the observation group had more significant improvement in the discomfort symptoms, with the score of (9.25±3.26) points, while the control group was (14.82±3.32) points. The allergic symptoms of the observation group were significantly alleviated, with the total effective rate reaching 83.3%, while that in the control group was only 66.7%. The improvement of pulmonary function in the observation group was more significant, with the total effective rate reaching 90.0%, while that in the control group was 76.6%, with statistically significant difference (P<0.05). Conclusion: Drug therapy for children with adenoidal hypertrophy of grade Ⅱ and above is more suitable, especially for children under 6 years. Surgical resection of adenoids and tonsils should be considered in children with poor outcomes and confirmed by OSA-18 score as having the serious impacts on quality of life.
