肺表面活性物质气管灌洗、气管滴注联合高频振荡通气在新生儿胎粪吸入综合征中的应用

摘要:
目的:探讨肺表面活性物质(PS)气管灌洗、气管滴注联合高频振荡通气在新生儿胎粪吸入综合征(MAS)中的应用。方法:回顾性分析60例新生儿MAS临床资料,振荡组28例患儿给予基础治疗+高频振荡通气治疗,联合组32例患儿均给予PS气管灌洗、气管滴注+高频震荡通气+基础治疗,比较两组临床症状好转时间、治疗前后血气分析和肺氧合功能指标、疗效、并发症发生率。结果:治疗后联合组患儿临床症状好转时间均短于振荡组(P<0.05),两组血气分析和氧合功能指标均改善(P<0.05),联合组优于振荡组(P<0.05)。联合组总有效率高于振荡组(P<0.05),并发症发生率低于振荡组(P<0.05)。结论:对新生儿MAS给予PS气管灌洗、气管滴注联合高频震荡通气疗效显著,能够促进症状好转,改善血气分析指标和肺氧合功能,减少并发症。

Abstract:
Objective: To explore the application of pulmonary surfactant (PS) tracheal lavage, tracheal instillation combined with high frequency oscillatory ventilation in neonatal meconium aspiration syndrome (MAS). Methods: Clinical data of 60 neonates with MAS were retrospectively analyzed. Totally 28 children in the oscillatory group were given basic therapy + high frequency oscillatory ventilation, and 32 children in the combined group received PS tracheal lavage, tracheal drip + high frequency oscillatory ventilation + basic therapy. The improvement time of clinical symptoms, blood gas analysis and lung oxygenation function indexes before and after treatment, efficacy and complication rate were compared between two groups. Results: After treatment, the improvement time of clinical symptoms in the combined group was shorter than that in the oscillatory group (P<0.05). Blood gas analysis and oxygenation function indexes in both groups were improved (P<0.05), and the combined group was better than the oscillatory group (P<0.05). The total effective rate of the combined group was higher than that of the oscillatory group (P<0.05), and the complication rate was lower than that of the oscillatory group (P<0.05). Conclusion: The efficacy of PS tracheal lavage, tracheal instillation combined with high frequency oscillatory ventilation for neonatal MAS is significant, which can promote the improvement of symptoms, improve the blood gas analysis indicators and lung oxygenation function, and reduce the complications of neonatal MAS.
