舒芬太尼联合地塞米松在小儿术后静脉自控镇痛的应用

摘要:
目的:探讨舒芬太尼联合地塞米松在小儿术后静脉自控镇痛的应用价值。方法:选择ASA Ⅰ~Ⅱ级择期手术全身麻醉术后患儿84例,随机分为对照组和试验组各42例。对照组采用舒芬太尼进行术后镇痛,试验组采用舒芬太尼联合地塞米松,观察两组患儿术后2 h、6 h、12 h、24 h、48 h生命体征指标,如呼吸频率(RR)、心率(HR)、平均动脉压(MAP)、血氧饱和度(SPO2)等。采用视觉模拟评分法(VAS)、最新改良镇静评分(Ramsay评分)评价术后药物的镇静镇痛效果,并记录术后药物不良反应。结果:两组患儿术后各时点的RR、MAP、HR、SpO2比较差异均无统计学意义(P均>0.05)。试验组患儿的术后VAS评分低于对照组,而Ramsay评分高于对照组,且呼吸抑制、皮肤瘙痒、恶心呕吐、眩晕及尿潴留等不良反应发生率均低于对照组(P均<0.01)。结论:与单用舒芬太尼进行术后镇痛相比,舒芬太尼联合地塞米松可显著提高镇痛镇静效果,且对呼吸抑制作用较弱,不良反应较少。

Abstract:
Objective: To explore the clinical application value of sufentanil combined with dexamethasone in the treatment of postoperative intravenous self-controlled analgesia in children. Methods: Totally 84 children with general anesthesia of American Society of Anesthesiologists (ASA) Ⅰ to Ⅱ were randomly divided into the control group and the experimental group, with 42 cases in each group. The control group was treated with sufentanil, while the experimental group received sufentanil combined with dexamethasone. The vital signs such as respiratory rate (RR), heart rate (HR), mean arterial pressure (MAP) and blood oxygen saturation (SpO2) were observed after surgery of 2 h, 6 h, 12 h, 24 h and 48 h. Visual analogue scale (VAS) and latest modified sedation score (Ramsay) were used to evaluate the sedative and analgesic effects of postoperative drugs, and adverse drug reactions were recorded. Results: There was no significant difference in RR, MAP, HR and SpO2 between two groups (P>0.05). Postoperative VAS score and Ramsay score in the experimental group were lower than those in the control group, and incidence of adverse drug reactions such as respiratory inhibition, skin itching, nausea and vomiting, dizziness and urinary retention were lower than those in the control group (P<0.01). Conclusion: Compared with sufentanil alone for postoperative analgesia, sufentanil combined with dexamethasone can significantly improve the analgesic and sedative effects, and has weaker respiratory depression and fewer adverse drug reactions.
