B型钠尿肽在小儿重症肺炎诊疗及预后评价中的应用

摘要:
目的:探讨小儿重症肺炎患者血浆B型钠尿肽(BNP)表达水平在疾病诊疗和预后评价中的价值。方法:将120例于我院就诊的重症肺炎患儿作为研究对象,根据临床结局分为痊愈出院组(A组,n=92)和死亡组(B组,n=28),回顾性分析两组患儿治疗前的血浆BNP水平、超敏C反应蛋白(hsCRP)、动脉血氧分压(PO2)以及急性生理和慢性健康状况评分(APACHE II)。分析患儿血浆BNP与病情以及预后的相关性。结果:A组和B组患儿的血浆BNP分别为(86.65±37.44)pg/mL和(128.55±42.86)pg/mL,hsCRP分别为(1.89±0.45)mg/L和(3.11±0.62)mg/L,PO2分别为(56.07±7.85)mm Hg和(52.38±8.14)mm Hg,APACHE II分别为(17.53±5.54)分和(20.89±7.22)分,B组患儿的血浆BNP、hsCRP和APACHE II明显高于A组,PO2明显低于A组,以上指标比较差异均有统计学意义(均P<0.05);患儿血浆BNP水平与hsCRP水平呈正相关(r=0.672,P=0.025),与APACHE II评分呈正相关(r=0.732,P=0.036),与PO2呈负相关(r=-0.511,P=0.046);血浆BNP预测患儿死亡结局的ROC曲线下面积为0.96。结论:血浆BNP在小儿重症肺炎的疾病诊疗和预后评价中有较好的临床应用价值。

Abstract:
Objective: To explore the value of B-type natriuretic peptide (BNP) in diagnosis, treatment and prognosis evaluation of children with severe pneumonia. Methods: 120 cases of children with severe pneumonia in our hospital were obtained as research objects, according to the clinical outcomes they were divided into treatment group (Group A, n=92) and death group (Group B, n=28). Retrospective analysis of plasma BNP levels, hypersensitive c-reactive protein (hsCRP), arterial blood oxygen pressure (PO2) and acute physiology and chronic health evaluation (APACHE II) before treatment of two groups was executed. The correlation between plasma BNP and illness, prognosis was analyzed. Results: The plasma BNP of group A and group B was (86.65±37.44) pg/mL and (128.55±42.86) pg/mL respectively, hsCRP was (1.89±0.45) mg/L and (3.11±0.62) mg/L respectively, PO2 was (56.07±7.85) mmHg and (52.38±8.14) mmHg respectively, APACHE II was (17.53±5.54) and (20.89±7.22) respectively, the plasma BNP, hsCRP and APACHE II of Group B were significantly higher than group A, the PO2 was lower than group A, the above differences were statistically significant (all P<0.05); the plasma BNP level of children was positively correlated with hsCRP level (r=0.672, P=0.672) and APACHE II scores (r=0.732,P=0.036), was negatively correlated with PO2 (r=-0.511, P=0.511); The area under the ROC curve for death ending of Plasma BNP prediction was 0.96. Conclusion: The plasma BNP has good clinical application value in the assessment of diagnosis, treatment and prognosis of children with severe pneumonia.
