136例新生儿脑积水临床分析

摘要:
目的:探讨新生儿脑积水的临床特点。方法:应用儿科住院病历检索系统,检索2013年6月至2018年6月在重庆医科大学附属儿童医院新生儿科确诊的脑积水患儿136例,分为早产儿组78例和足月儿组58例。对两组患儿的临床特点、病因构成、治疗等进行回顾性分析。结果:早产儿组的颅内出血、新生儿窒息及凝血功能障碍发生率均高于足月儿组,而颅内感染、痫性发作发生率均低于足月儿组(P均<0.05);两组在重度窒息、脑积水严重程度方面比较差异无统计学意义(P>0.05)。结论:颅内出血是新生儿脑积水的首要病因,尤其早产儿更容易发生颅内出血、窒息、凝血功能障碍等,而在足月儿中还需警惕颅内感染。此外,应积极处理重度脑积水,以减轻其对神经系统发育产生的不良影响

Abstract:
Objective: To probe into the clinical characteristics of hydrocephalus in neonates. Methods: The pediatric inpatient medical records retrieval system was retrieved, 136 children with neonatal hydrocephalus in the neonatology department of Children’s Hospital of Chongqing Medical University from Jun. 2013 to Jun. 2018 were searched, which were divided into the premature infant group (n=78) and the term infant group (n=58). The clinical characteristics, etiology and therapy were retrospectively analyzed. Results: The incidence of intracranial hemorrhage, neonatal asphyxia and coagulation dysfunction in the premature infant group was higher than that in the term infant group, while the incidence of intracranial infection and epileptic seizure were lower than that in the term infant group (P<0.05). There was no significant difference in the severity of severe asphyxia and hydrocephalus between two groups (P>0.05). Conclusion: Intracranial hemorrhage is the primary cause of neonatal hydrocephalus, and premature infants are more prone to have intracranial hemorrhage, asphyxia, coagulation dysfunction, etc. In term infants, it is also necessary to alert intracranial infection. In addition, severe neonatal hydrocephalus should be actively managed to reduce its adverse effects on the development of nervous system.
