舒芬太尼联合依托咪酯对小儿先天性心脏病手术麻醉质量及血流动力学的影响

摘要:
目的:探讨舒芬太尼联合依托咪酯在小儿先心脏病手术麻醉中的应用价值。方法:选取我院2016年6月至2018年9月拟进行小儿先天性心脏病手术的患儿96例,随机分为试验组和对照组各48例。对照组采用舒芬太尼麻醉,试验组采用舒芬太尼联合依托咪酯麻醉,比较两组患儿麻醉前(t1)、麻醉诱导后10 min(t2)、插管时(t3)、切皮时(t4)、锯胸骨后(t5)、术毕(t6)6个时间点的心率(HR)、平均动脉压(MAP)、收缩压(SBP)及舒张压(DBP),并以躁动评分、术后苏醒时间及拔管时间来评价麻醉效果,同时观察术后24 h不良反应的发生情况。结果:两组患儿麻醉前HR、MAP、SBP及DBP比较差异均无统计学意义(P均>0.05),围术期(t1~t6)HR比较差异均无统计学意义(P均>0.05)。两组患儿MAP、SBP及DBP在麻醉诱导及插管后均有下降趋势,试验组下降幅度较小。试验组患儿躁动评分、苏醒时间、拔管时间及术后不良反应发生率均低于或短于对照组(P均<0.05)。结论:在小儿先天性心脏病手术中应用舒芬太尼联合依托咪酯静脉麻醉效果稳定,安全性好,能够提高麻醉质量。

Abstract:
Objective: To investigate the application value of sufentanil combined with etomidate in anesthesia for children with congenital heart disease. Methods: Totally 96 children with congenital heart disease admitted into our hospital from Jun. 2016 to Sept. 2018 were randomly divided into the experimental group and the control group, with 48 cases in each group. The control group was treated with sufentanil and the experimental group received sufentanil combined with etomidate. Heart rate (HR), mean arterial pressure (MAP), systolic blood pressure (SBP) and diastolic blood pressure (DBP) before anesthesia (t1), 10 min after anesthesia induction (t2), intubation (t3), skin incision (t4), posterior sternum (t5) and after surgery (t6) of two groups were compared. Anesthesia effect was evaluated by agitation score, postoperative recovery time and extubation time. The incidence of adverse drug reactions was observed after surgery of 24 h. Results: There was no significant difference in HR, MAP, SBP and DBP between two groups (P>0.05). There was no significant difference in HR during the perioperative period (t1~t6, P>0.05). The MAP, SBP and DBP of two groups had a downward trend after induction of anesthesia and intubation, and the decline in the experimental group was smaller. The agitation score, recovery time, extubation time and incidence of postoperative adverse drug reactions in the experimental group were lower than or shorter than those in the control group (P<0.05). Conclusion: The anesthesia effect of sufentanil combined with etomidate for pediatric congenital heart disease is stable and safe, sufentanil combined with etomidate can improve the anesthesia quality of surgery.
