2017年某院儿科超说明书用药调查

摘要:
目的:调查2017年某院儿科超说明书用药情况,为促进儿科合理用药提供参考。方法:回顾性分析2017年奉城医院儿科门急诊处方及住院医嘱,根据最新版药品说明书,判断超说明书用药情况,并对超说明书用药类型、患儿年龄段和医师职称等进行统计分析。结果:我院2017年儿科门急诊处方及住院医嘱超说明书用药占比分别为1.44%、1.30%,涉及口服药32种,注射剂16种。门急诊处方及住院医嘱超说明书用药类型主要为无儿童用药信息(70.95%、68.21%)、超年龄用药(10.24%、13.91%)及超适应症用药(8.30%、9.27%)。排名前5位的超说明书用药药品分别为注射用单磷酸阿糖腺苷、注射用甲泼尼龙琥珀酸钠、利巴韦林注射液、阿昔洛韦、盐酸氨溴索注射液。患儿年龄越大,门急诊处方及住院医嘱中超说明书用药的发生率越低(P<0.05),医师职称越高,超说明书用药发生率越高(P<0.05)。结论:奉城医院门急诊处方及住院医嘱均存在一定超说明书用药情况,应加强药物使用的规范管理,强化医务人员培训,尽量减少或避免超说明书用药,保障儿童用药的合理性与安全性。

Investigation of Off-Label Drug Use in Pediatrics of a Hospital in 2017

Abstract:
Objective: To investigate the off-label drug use in pediatrics of a hospital in 2017, so as to promote the rational drug use in pediatrics. Methods: Retrospective analysis was conducted on pediatric outpatient and emergency prescriptions and inpatient medical orders in Fengcheng Hospital in 2017. Off-label drug use was judged according to the latest drug instructions, and statistical analysis was conducted on types, age of children and physician titles of off-label drug use. Results: In 2017, the proportion of off-label drug use in pediatric outpatient and emergency prescriptions and inpatient medical orders of our hospital was respectively 1.44% and 1.30%, including 32 kinds of oral drugs and 16 kinds of injections. Off-label drug use in pediatric outpatient and emergency prescriptions and inpatient medical orders mainly focused on without drug use information in children (70.95%, 68.21%), off-age drug use (10.24%, 13.91%), and over indications (8.30%, 9.27%). The top five off-label drugs were vidarabine injection, meprednisolone injection, ribavirin injection, acyclovir and ambroxol injection. With the increase of age, the incidence of off-label drug use in outpatient and emergency prescriptions and inpatient medical orders decreased significantly (P<0.05), and the higher the professional title, the higher the incidence of off-label drug use (P<0.05). Conclusion: There were off-label drug use in outpatient and emergency prescriptions and inpatient medical orders of Fengcheng Hospital. Therefore, standardized management of drug use and medical staff training should be strengthened to minimize or avoid off-label drug use and ensure the rationality and safety of drug use for children.
