维生素D对丙戊酸钠治疗小儿癫痫骨代谢异常不良反应的影响

摘要:
目的:探讨补充适量维生素D对丙戊酸钠治疗小儿癫痫引起的骨代谢异常不良反应的影响,为小儿癫痫的治疗提供参考。方法:选取我院2015年6月至2018年6月收治的60例癫痫患儿为研究对象,按随机数字表法分为联合组和对照组,对照组给予丙戊酸钠治疗,联合组在对照组治疗基础上适量补充维生素D,比较分析两组治疗前、治疗3个月后骨代谢指标水平的变化。结果:治疗前,两组患儿血钙(Ca)、血磷(P)、血清降钙素(CT)、维生素D3(VitD3)、碱性磷酸酶(ALP)、骨碱性磷酸酶(BAP)水平比较差异无统计学意义(P>0.05)。治疗3个月后,联合组Ca、VitD3、ALP水平均高于对照组(P<0.05),CT、BAP水平均低于对照组,差异有统计学意义(P<0.05);联合组P水平与对照组比较差异无统计学意义(P>0.05)。联合组治疗前后Ca、P、VitD3、CT、ALP水平比较差异无统计学意义(P>0.05),BAP水平比较差异有统计学意义(P<0.05)。治疗3个月后,对照组Ca、VitD3、ALP水平均低于治疗前(P<0.05),且治疗后CT、BAP水平高于治疗前,差异有统计学意义(P<0.05);对照组治疗前后P水平比较差异无统计学意义(P>0.05)。结论:补充适量维生素D联合丙戊酸钠治疗小儿癫痫的可减少骨代谢异常情况,为临床治疗小儿癫痫提供参考

Abstract:
Objective: To explore the effects of vitamin D supplementation on bone metabolism of sodium valproate in the treatment of children with epilepsy, so as to provide reference for the treatment of epilepsy in children. Methods: Sixty children with epilepsy admitted into our hospital from Jun. 2015 to Jun. 2018 were extracted to be divided into the combination group and the control group via the random number table, with 60 cases in each group. The control group was treated with sodium valproate, while the combination group additionally received vitamin D supplementation on the basis of the control group. Changes of bone metabolism indicators of two groups were compared and analyzed before treatment and after treatment of 3 months. Results: Before treatment, there was no significant difference in the levels of serum calcium (Ca), phosphate (P), calcitonin (CT), vitamin D3 (VitD3), alkaline phosphatase (ALP) and bone alkaline phosphatase (BAP) between two groups (P>0.05). After treatment of 3 months, the levels of Ca, VitD3 and ALP in the combination group were higher than those in the control group (P<0.05), and the levels of CT and BAP were lower than those in the control group, the differences were statistically significant (P<0.05). There was no significant difference in the levels of serum P between the combination group and the control group (P>0.05). There were no statistically significant difference in the levels of Ca, P, VitD3, CT and ALP in the combination group before and after treatment (P>0.05), while there was statistically significant difference in the levels of BAP (P<0.05). After treatment of 3 months, the levels of Ca, VitD3 and ALP in the control group were lower than those before treatment (P<0.05), and the levels of CT and BAP after treatment were higher than those before treatment, with statistically significant difference (P<0.05). There was no significant difference in the levels of serum P in the control group before and after treatment (P>0.05). Conclusion: Adequate vitamin D supplementation combined with sodium valproate in the treatment of pediatric epilepsy can reduce reduce the abnormal bone metabolism and provide reference for clinical treatment of pediatric epilepsy
