| 目的:回顾性统计分析郴州市第一人民医院注射用脂溶性维生素(Ⅰ)药品不良反应(ADR),探讨其所致ADR的一般规律及特点,为临床合理用药提供参考。方法:回顾性分析2010年1月至2019年12月我院上报至国家药品不良反应监测系统中注射用脂溶性维生素(Ⅰ)ADR病例的临床资料。结果:47例ADR,主要发生在1~3岁儿童,原发疾病主要为消化系统疾病(55.32%)。47例患儿均采用静脉滴注的方式给药,其中12例患儿存在超剂量给药,9例有合并用药。47例ADR多发生于30 min内,累及最多的系统/器官为皮肤及其附件损害,其次为心血管系统损害,常见临床表现为紫绀和皮疹。主要ADR类型为一般,预后良好。结论:为减少ADR发生,临床应合理应用注射用脂溶性维生素(Ⅰ),避免超说明书用药,减少药物联用。在用药过程中,应注意观察患者用药过程中临床反应,及时发现ADR并对症处理,避免严重ADR的发生。 |

| Objective: To retrospectively analyze the adverse drug reactions (ADR) induced by fat soluble vitamins (Ⅰ) for injection in Chenzhou First People’s Hospital, and to probe into the general rules and characteristics of ADR, so as to reference for rational drug use in clinic. Methods: Clinical data of ADR induced by fat soluble vitamins (Ⅰ) for injection reported to the national ADR monitoring system in our hospital from Jan. 2010 to Dec. 2019 were retrospectively analyzed. Results: Forty-seven cases of ADR mainly occurred in children aged from 1 to 3 years, and the primary diseases were mainly digestive system diseases (55.32%). Forty-seven cases were given intravenous drip infusion, 12 cases had off-label administration, and 9 cases had drug combination. Most of the 47 cases of ADR occurred within 30 min. The most frequently involved system/organ was the skin and its accessory damage, followed by the cardiovascular system damage, with common clinical manifestations of cyanosis and rash. The main type of ADR was general, and the prognosis was good. Conclusion: In order to reduce the incidence of ADR, fat soluble vitamins (Ⅰ) for injection should be used rationally in clinic to avoid off-label drug use and reduce the rate of drug combination. Attention should be paid to observe the clinical reaction of patients in the course of medication, timely detection of ADR and symptomatic treatment are necessary to avoid the occurrence of severe ADR. |
