早产儿应用枸橼酸咖啡因对体质量及炎症水平的影响

摘要:
目的:研究枸橼酸咖啡因对早产儿体质量及炎症水平的影响。方法:采用回顾性研究方法,选取深圳市人民医院新生儿科使用枸橼酸咖啡因的患儿,与未使用枸橼酸咖啡因病例匹配对照,比较两组患儿生后体质量及炎症水平的变化。结果:本研究共纳入使用枸橼酸咖啡因的患儿及未使用枸橼酸咖啡因患儿各27例。枸橼酸咖啡因组患儿出生体质量恢复时间(15.9±3.6)d,未使用枸橼酸咖啡因组患儿出生体质量恢复时间(10.2±3.6)d,两组比较差异有统计学意义(P<0.01)。枸橼酸咖啡因组患儿生后第7、10、14、18、21、25、28、32天的体质量均明显降低,而第4天及第33~35天两组患儿体质量比较差异无统计学意义。两组患儿各时间段白细胞水平比较差异无统计学意义。生后第19~21天枸橼酸咖啡因组患儿超敏C反应蛋白(hs-CRP)水平0.22 mg/L,未使用枸橼酸咖啡因组患儿hs-CRP水平0.10 mg/L,差异有统计学意义(P<0.05);余第1、7、14、28 d的hs-CRP水平比较差异无统计学意义(P>0.05)。结论:枸橼酸咖啡因的使用对早产儿早期体质量增长有显著影响,表现为恢复出生体质量时间延长及体质量增长迟缓。对此类早产儿需十分注意其营养管理,保证其体质量平稳增长。

Abstract:
Objective: To study the effects of caffeine citrate on body mass and inflammation levels in premature infants. Methods: Retrospective analysis was adopted, case-matched control was used to compare the postnatal mass and inflammation levels of two groups of children (with and without caffeine citrate) in neonates of Shenzhen People’s Hospital. Results: In this study, a total of 27 cases were included in the caffeine citrate group and 27 cases were included in the no caffeine citrate group. The recovery time of body mass in the caffeine citrate group was (15.9±3.6) d, while that of the the no caffeine citrate group was (10.2±3.6) d, with statistically significant difference between two groups (P<0.01). The body mass in the caffeine citrate group decreased significantly on the 7th, 10th, 14th, 18th, 21st, 25th, 28th and 32nd day after birth, while there was no significant difference in body mass between two groups on the 4th day and from the 33rd to 35th day. There was no statistically significant difference in the levels of white blood cells between two groups at various time periods. On the 19th to 21st day after birth, the hypersensitivity C-reactive protein (hs-CRP) level of children in the caffeine citrate group was 0.22 mg/L, and the hs-CRP level in the no caffeine citrate group was 0.10 mg/L, the difference was statistically significant (P<0.05). There was no statistically significant difference in hs-CRP level on the 1st, 7th, 14th, and 28th day (P>0.05). Conclusion: The use of caffeine citrate has a significant effect on the body mass gain of premature infants in the early stage, which is manifested as prolonged recovery time of birth body mass and slow body mass gain. Attention should be paid to nutrition management for premature infants to ensure the steady body mass gain.
