基于真实世界数据挖掘的儿童中成药剂量模型研究

摘要:
目的:建立无儿童剂量中成药的儿童用量模型,为儿童临床合理使用中成药提供参考。方法:回顾性调查成都市妇女儿童中心医院2019年1月1日至6月30日急诊药房处方,以Excel 2007建立数据库,采用SPSS 19.0进行统计分析,以频数分析法对年龄、临床诊断、用法用量等进行统计分析,在患儿中成药用量统计结果中寻找规律,建立真实世界儿童中成药剂量模型,并对模型进行验证。结果:最终纳入20 833张处方,对使用率排名前三的中成药蒲地蓝消炎口服液、橘半止咳颗粒、柴黄颗粒在不同年龄阶段儿童中使用的剂量进行统计分析,运用Excel 2007建立适用于这三种药物的中成药儿童剂量模型,最小剂量公式:X=IF(N<0.9,M/6,IF(N<1,M/3,IF(N<3,M/3IF(N<6,M/2,IF(N<=10,M/3*2)))));最大剂量公式:Y=IF(N<0.9,M/6,IF(N<1,M/3,IF(N<3,M/2,IF(N<6,M/3*2,IF(N<=10.M)))))。结论:儿童临床使用非专用中成药情况较为普遍,为降低儿童中成药的使用风险,本文建立了一种数学模型,规范儿童中成药的用法用量,为临床需要提供一个适宜的用药方案,易于临床医师开具处方、药师审查处方,同时也利于患者及家属掌握中成药的使用方法。

Dosage Model of Chinese Patent Medicine for Children Based on Real World Data Mining

Abstract:
Objective: To establish the dosage model of Chinese patent medicine for children, so as to provide reference for clinical rational use of Chinese patent medicine for children. Methods: Retrospective investigation was performed on prescriptions in the emergency pharmacy of Chengdu Women’s and Children’s Central Hospital from Jan. 1st to Jun. 30th, 2019. The database was created in Excel 2007, and SPSS 19.0 was used for statistical analysis. Age, clinical diagnosis and dosage were analyzed by frequency analysis. The rules were found in the statistical results of the dosage of Chinese patent medicine for children. The dosage model of Chinese patent medicine for children in the real world was established and verified. Results: Totally 20,833 prescriptions were included. Statistical analysis was performed on Pudilan Xiaoyan oral liquid, Juban Zhike granules and Chaihuang granules (the top three dosage of Chinese patent medicine) used in children of different age groups. Excel 2007 was used to establish the dosage model of Chinese patent medicine for children. Minimum dosage formula: X=IF (N<0.9, M/6, IF (N<1, M/3, IF (N<3, M/3, IF (N<6, M/2, IF (N<=10, M/3*2))))); maximum dosage formula: Y=IF (N<0.9, M/6, IF (N<1, M/3, IF (N<3, M/2, IF (N<6, M/3*2, IF (N<=10, M))))). Conclusion: The clinical use of non-specific Chinese patent medicine for children is relatively common. In order to reduce the risk of using Chinese patent medicine for children, this paper establishes the mathematical model to standardize the dosage of Chinese patent medicine for children and provide appropriate medication regimen of Chinese patent medicine for the clinic, which is easy for clinicians to prescribe and pharmacists to review prescriptions, and also help the patients and the families to master the use of Chinese patent medicine.
