临床药师对1例超低出生体质量早产儿合并代谢性骨病的药学监护

摘要:
[摘要]目的:探讨临床药师参与1例超低出生体质量早产儿合并代谢性骨病的钙、磷补充及个体化肠外营养方案制定的切入点。方法:临床药师参与1例超低出生体质量早产儿合并代谢性骨病患儿的治疗过程,并对治疗方案与监测指标进行分析和总结。结果:结合患儿病理生理特点和病情进展,临床药师对患儿肠外营养钙、磷补充及维生素D添加提出药学建议,参与肠外营养个体化用药方案制定,促进疾病转归。结论:临床药师从药学角度参与早产儿个体化药物治疗,尤其是是肠外营养方案的优化及加强代谢性骨病相关药学监护,在促进临床合理用药、改善患儿预后方面发挥积极作用。

Abstract:
[Abstract] Objective: To explore the entry points of clinical pharmacists to participate into the calcium and phosphorus supplementation and individualized parenteral nutrition programs for extremely low birth weight (ELBW) preterm infants with metabolic bone disease. Methods: Clinical pharmacists participated into the treatment process of an ELBW preterm infant with metabolic bone disease, the treatment regimen and monitoring indicators were analyzed and summarized. Results: Combined with the pathophysiological characteristics and disease progression of the infant, clinical pharmacists provided pharmaceutical suggestions for calcium, phosphorus and vitamin D supplementation of parenteral nutrition in children, participated into the formulation of individualized parenteral nutrition programs, and promoted the disease outcomes. Conclusion: Clinical pharmacists participate into the individualized drug treatment of preterm infant from the perspective of pharmacy, especially the optimization of parenteral nutrition programs and the strengthening of pharmaceutical care related to metabolic bone disease, which play an active role in promoting clinical rational drug use and improving the prognosis of children.
