目的:探讨湖南省儿童医院新生儿抗感染治疗存在的问题并总结临床药师参与新生儿抗感染会诊的经验和效果。方法:对2019年5月至2020年5月我院临床药师参与的310例次新生儿科抗感染会诊进行统计、分析。结果:2019年5月至2020年5月我院临床药师共参与新生儿科抗感染会诊310例次。排名前三的会诊目的依次为初始治疗疗效不佳需调整方案174例次(54.89%),协助区分污染、定植或致病菌(是否需要抗感染治疗)37例次(11.67%)及抗真菌药物选用36例次(11.36%)。临床药师给出会诊意见排名前三依次为调整抗菌药物给药方案183例次(57.73%)、续用或优化原药物方案71例次(22.40%)、结合临床综合评价是否使用抗菌药物37例次(11.67%)。会诊意见被临床全部采纳273例次(88.06%)。疾病转归为有效237例次,有效率86.81%(237/273)。结论:我院医师在新生儿抗感染治疗方面存在一定困惑,临床药师通过参与临床会诊,为医师及患儿提供药学服务对促进患儿疾病诊治和合理用药具有积极作用。 |

Anti-Infection Consultations in 310 Neonates by Clinical Pharmacists |

Abstract: |
Objective: To probe into the problems of anti-infection treatment for neonates in Hunan Children’s Hospital and summarize the experience and effect of anti-infection consultations for neonates by clinical pharmacists. Methods: From May 2019 to May 2020, 310 anti-infection consultations in neonatology participated by clinical pharmacists in our hospital were statistically analyzed. Results: From May 2019 to May 2020, clinical pharmacists participated into 310 anti-infection consultations in neonatology. The top three consultation purposes were 174 cases (54.89%) of poor initial therapeutic efficacy requiring adjustment of regimen, 37 cases (11.67%) of assistance in identifying contaminated, colonized or pathogenic bacteria (whether anti-infection treatment was needed or not) and 36 cases (11.36%) of antifungal drug selection. The top three consultation opinions given by clinical pharmacists were 183 cases (57.73%) of adjustment of antibacterial drug administration regimen, 71 cases (22.40%) of continuation or optimization of original drug administration regimen, and 37 cases (11.67%) of comprehensive evaluation on use of antibacterial drugs combined with clinical practice. The consultation opinion was adopted by 273 cases (88.06%) clinically. The disease turned out to be effective in 237 cases, with an effective rate of 86.81% (237/273). Conclusion: Clinical pharmacists provide pharmaceutical care for clinicians and children through participating into the clinical consultations, which plays a positive role in promoting diagnosis and treatment of diseases and rational drug use in children, as there is some confusion among clinicians in neonatal anti-infection treatment. |
