| [摘要]目的：探讨促肾上腺皮质激素（ACTH）治疗癫痫性痉挛的药学监护方法及不良事件的应对措施。方法：2例患儿采用ACTH治疗前中性粒细胞/淋巴细胞比值均<0.5，且前白蛋白（0.25 g/L）异常增高，属于ACTH治疗引起高血压的高风险人群。临床药师提前制定药学监护方案，由医师及临床药师共同开展24 h药学监护，保障患儿治疗安全。结果：2例患儿使用ACTH期间均出现高血压，给予卡托普利抗高血压后好转。此外，1例出现血小板增高（反应性，自行好转），另1例出现咳嗽（雾化对症治疗后好转）。结论：ACTH治疗癫痫性痉挛期间应重点关注血压和免疫相关的药学监护。 |

| [Abstract] Objective: To probe into the pharmaceutical care method of adrenocorticotropic hormone (ACTH) in the treatment of epileptic spasm and the countermeasures of adverse events. Methods: Before ACTH treatment, the neutrophil/lymphocyte ratio of two children were less than 0.5, and the prealbumin (0.25 g/L) increased abnormally, which indicated these children might associate with high risk of hypertension during ACTH treatment. The clinical pharmacist formulated pharmaceutical care regimen in advance, the clinicians and clinical pharmacists jointly carried out 24 h pharmaceutical care to ensure the safety of treatment. Results: Hypertension were developed in both children during ACTH treatment and were improved with the administration of captopril. In addition, one case presented with thrombocytosis (reactive and improved) and one case with cough (improved after symptomatic treatment with nebulization). Conclusion: Attention should be paid to blood pressure and immune-related pharmaceutical care during the treatment of epileptic spasm with ACTH. |
