| 目的:探讨新生儿甲型流行性感冒的临床特点、治疗及预后。方法:回顾分析2018 年1 月至2020 年1 月湖南省儿童医
院新生儿科收治的诊断为甲型流行性感冒的25 例住院新生儿临床资料,对患儿的临床特点、辅助检查、治疗过程及疾病转归进
行分析。结果:25 例患儿中,男17 例,女8 例 。16 例足月儿,9 例早产儿,其中7 例为晚期早产儿。18 例冬春季发病,7 例夏季
发病。12 例发病前有“感冒”症状家人密切接触史。临床表现咳嗽17 例,发热17 例,呼吸困难10 例,鼻塞12 例,流涕3 例,呼
吸暂停1 例。25 例患儿甲型流感病毒抗原检测均为阳性,6 例患儿同时呼吸道合胞病毒检测阳性,17 例痰细菌培养阳性。
16 例使用奥司他韦治疗,2 例使用帕拉米韦治疗。24 例治愈出院,1 例死亡,死亡患儿为早产儿。结论:新生儿甲型流感症状多
不典型,冬季和夏季发病多见,预后大多较好,但早产儿需警惕严重并发症及不良预后。 |

| Objective: To explore the clinical characteristics, treatment and prognosis of neonatal influenza A. Methods: Clinical data
of 25 hospitalized neonates diagnosed with influenza A in Neonatology Department of Hunan Children’s Hospital from January 2018 to
January 2020 were analyzed retrospectively. The clinical characteristics, auxiliary examinations, treatment process and outcomes of the neonates were analyzed. Results: Among the 25 cases, 17 were male and 8 were female. There were 16 full-term infants and 9
premature infants, among which 7 were late premature infants. Eighteen cases occurred in winter and spring, and 7 cases occurred in
summer. Twelve cases had close contact with family members with symptoms of “cold” before ill. Clinical manifestations included cough
in 17 cases, fever in 17 cases, dyspnea in 10 cases, nasal congestion in 12 cases, runny nose in 3 cases, and apnea in 1 case. All the
25 cases tested positive for influenza A virus antigen, 6 cases tested positive for respiratory syncytial virus at the same time, and sputum
bacteria culture was positive in 17 cases. Sixteen cases were treated with oseltamivir and 2 cases with peramivir. Twenty-four cases were
cured and discharged, and 1 case died (a premature infant). Conclusion: The symptoms of influenza A infection in neonates are not
typical, the incidence is usually high in winter and summer, and the prognosis is generally good, but the premature infants should be
alert to serious complications and poor prognosis. |
