104 例新生儿地高辛的应用与治疗药物监测

摘要:
目的:分析我院新生儿地高辛的使用与治疗药物监测( TDM) 情况,为临床合理用药提供参考。方法:收集我院2016- 2020 年应用地高辛的新生儿病例,对患儿一般资料、地高辛用药剂量、给药频率、给药途经、治疗药物监测情况及临床结局进行 回顾性分析。结果:共纳入104 例应用地高辛的住院新生儿,其中早产儿43 例,足月儿61 例,维持剂量分别为(7. 0±2. 4)、(8. 2± 1. 5)μg/ (kg·d)。约96. 2%的患儿采用q12h 给药,93. 3%患儿口服给药。78 例(75. 0%)患儿开展了TDM,以0. 5~2. 0 ng/ mL 为谷浓度参考范围,有25 例(24. 0%)患儿血浆药物浓度超出参考范围上限,无低于参考范围下限的病例。早产儿与足月儿按 推荐剂量给药时血药浓度均为(1. 7±0. 7)ng/ mL,临床治愈率分别为50. 0%和61. 8%。结论:新生儿尤其早产儿对地高辛较为 敏感,建议密切监测药物浓度与不良反应,临床药师可提供新生儿地高辛个体化药学服务,促进临床合理用药。

Abstract:
Objective: To analyze the therapeutic drug monitoring and treatment situation of digoxin in neonates and provide references for clinical rational use of digoxin. Methods: The neonatal cases using digoxin in our hospital from 2016 to 2020 were collected, and the patient data, digoxin dosage, administration frequency, routes, therapeutic drug monitoring and clinical outcomes were retrospectively analyzed. Results: A total of 104 hospitalized neonates treated with digoxin were enrolled, including 43 preterm infants and 61 term infants and the average maintenance dose of digoxin was (7. 0±2. 4) and (8. 2±1. 5) μg/ (kg·d), respectively. About 96. 2% of the children were administered with q12h frequency, and 93. 3% were administered orally. And 78 children underwent therapeutic drug monitoring, with 0. 5 to 2. 0 ng/ mL as the reference range of trough concentration, among which, the drug concentrations of 25(24. 0%) children exceeded the upper limit of the reference value, and no case was lower than the lower limit of the reference value. The average drug concentration of preterm infants and full-term infants at the recommended doses were both (1. 7±0. 7) ng/ mL, respectively, and the clinical cure rates were 50. 0% and 61. 8%, respectively. Conclusion: Neonates, especially premature infants, are sensitive to digoxin. It is recommended to closely monitor the drug concentration and adverse reactions. Clinical pharmacists can provide individualized pharmaceutical services for neonates to promote clinical rational drug use.
