槐杞黄颗粒对轻度持续期哮喘患儿小气道功能及免疫功能的影响

摘要:
目的:探讨槐杞黄颗粒在轻度持续期哮喘患儿的应用价值。方法:采用前瞻性随机对照研究方法,收集我院儿科门诊轻度持续期哮喘患儿60 例,按随机数表法分为槐杞黄组和对照组各30 例。对照组采用吸入性糖皮质激素(ICS) 治疗,槐杞黄组采用ICS+槐杞黄颗粒治疗,比较两组患儿治疗后1、3、6 个月的临床评价指标及儿童哮喘控制测试(C-ACT)评分,并监测治疗前后免疫球蛋白E(IgE)、白细胞介素(IL)-4、IL-17、呼出气一氧化氮(FeNO)、脉冲震荡肺功能(IOS)水平的变化。结果:与对照组相比,槐杞黄组治疗后3、6 个月临床症状改善较明显且C-ACT 评分较高(P 均<0.05)。治疗3 个月后,两组患儿IgE、IL-4、IL-17、FeNO、气道总阻力、外周弹性阻力、共振频率水平较治疗前降低,且槐杞黄组均低于对照组(P 均<0. 05);两组中心气道阻力与治疗前比较差异无统计学意义(P>0. 05)。结论:槐杞黄颗粒联合ICS 治疗儿童轻度持续期哮喘疗效更佳,可下调IgE、IL-4、IL-17 水平,减轻气道炎症,改善小气道功能。

Effects of Huaiqihuang granules on Small Airway Function and Immune Function in Children with Mild Persistent Asthma

Abstract:
Objective: To study the application value of Huaiqihuang granules in children with mild persistent asthma. Methods: A prospective, randomized, controlled study method was used to collect 60 cases of children with mild persistent asthma who attended the pediatric clinic of our hospital. They were divided into two groups according to the random number table method, including the Huaiqihuang group ( treated with inhaled corticosteroids + Huaiqihuang granules) and the control group ( treated with inhaled corticosteroids), 30 cases for each. Comparing the clinical evaluation indicators and child asthma control Test (C-ACT) scores at 1, 3 and 6 months after treatment, and monitoring the changes of IgE, IL-4, IL-17, exhaled nitric oxide (FeNO), and impulsed pulmonary function (IOS) before and after treatment. Results: After 3, 6 months of treatment in the two groups, the clinical symptoms of the Huaiqihuang group were significantly improved compared with those of the control group , and the C-ACT was the highest (P<0. 05). After 3 months of treatment, the levels of IgE, IL-4, IL-17, FeNO, total airway resistance, peripheral elastic resistance and resonance frequency in the two groups were significantly lower than before treatment, with the Huaiqihuang group significantly lower than control group (P<0. 05). There was no significant difference in central airway resistance in two groups compared with that before the treatment (P>0. 05). Conclusion: Huaiqihuang granules combined with inhaled corticosteroids has a better therapeutic effect on asthma, and can reduce airway inflammation, lower the level of eosinophils and improve small airway function by down-regulating IgE, IL-4, and IL-17.
