ω-3 脂肪酸对自闭症谱系障碍患儿干预效果的 Meta 分析

摘要:
目的:了解 ω-3 脂肪酸对自闭症谱系障碍(ASD) 患儿的干预效果。 方法:系统检索 PubMed、EMbase、the Cochrane Library、Web of Science、SinoMed、中国知网、万方、维普等数据库中关于 ω-3 脂肪酸治疗儿童及青少年 ASD 的随机对照试验,检索时间为建库至 2021 年 2 月,并筛选符合纳入标准的试验,使用 RevMan 5. 3 软件进行统计分析。 结果:纳入 8 项随机对照试验,共计 340 例 ASD 患儿。 与对照组比较,试验组补充 ω-3 脂肪酸可改善 ASD 患儿的多动症状(MD = -2. 60,95%CI -4. 92 ~-0. 27,P< 0. 05)、社会退缩/ 呆滞症状(MD = - 2. 17,95% CI - 3. 65 ~ - 0. 69,P< 0. 05)。 与试验组比较,对照组外化行为评分(MD= 6. 22,95%CI 1. 59~10. 86,P<0. 05)改善明显。 结论:补充 ω-3 脂肪酸可改善 ASD 患儿的多动及社会退缩/ 呆滞症状,副作用小、耐受性好,但补充 ω-3 脂肪酸对 ASD 患儿的整体临床疗效及具体剂量的选择仍需要更多高质量、多中心、大样本量的随机对照试验来进行验证。

Abstract:
Objective: To investigate the intervention effects of ω-3 fatty acids on children with autism spectrum disorder (ASD). Methods: PubMed, EMBase, the Cochrane Library, Web of Science, SinoMed, CNKI, Wanfang and VIP databases were systematically retrieved for randomized controlled trials on ω-3 fatty acid treatment for children and adolescents with ASD. The retrieval time was from the establishment of the database to Feb. 2021. Trials that met the inclusion criteria were screened and statistically analyzed by using RevMan 5. 3 software. Results: Eight randomized controlled trials were enrolled, including 340 children with ASD. Compared with the control group, the experimental group showed significant improvements in symptoms of hyperactivity (MD = -2. 60, 95%CI from -4. 92 to -0. 27, P<0. 05) and social withdrawal/ autism symptoms (MD = -2. 17, 95%CI from -3. 65 to -0. 69, P <0. 05). Compared with the experimental group, the control group showed significant improvements in the externalizing behavior score (MD= 6. 22, 95%CI from 1. 59 to 10. 86, P<0. 05) after ω-3 fatty acid supplementation. Conclusion: ω-3 fatty acid supplementation can improve symptoms of hyperactivity and social withdrawal/ autism in children with ASD, which has minimal side effects and good tolerability. However, further high-quality, multi-center, large-scale randomized controlled trials are needed to confirm the overall clinical efficacy and optimal dosage selection for ω-3 fatty acid supplementation in children with ASD.
