| 目的:了解国内儿童(妇幼)专科医院静脉用药调配中心(PIVAS)运行现状,为进一步规范 PIVAS 工作模式提供参考。 方 法:采用问卷调查的方法,对全国 16 家儿童(妇幼)专科医院 PIVAS 负责人进行调查,并对问卷结果进行统计分析。 结果:16 家 PIVAS 建设面积 80~800 m2 ,人均每日调配量 17 ~ 200 组。 12 家 PIVAS 人员构成是药护结合模式,4 家是单药师模式。 16 家 PIVAS 均开展静脉医嘱审核,其中 4 家由临床药师审方。 PIVAS 工作范围包括大部分科室和药物类型,医嘱类型除静脉滴注 外,还包括静脉推注、镇痛泵等。 仅 4 家 PIVAS 配备了自动配药机器人,自动化程度较低。 16 家 PIVAS 普遍存在分剂量调配、 药品合用、药品结余现象。 PIVAS 调配费各地区之间差异较大,每组的标准为肠外营养液 15. 00 ~ 90. 00 元、细胞毒药物 8. 00 ~ 40. 00 元、抗菌药物 0~10. 00 元、普通药物 0 ~ 9. 00 元。 结论:儿童(妇幼)专科医院 PIVAS 工作模式有其特点,但也存在数量 少、专业人员短缺、工作负荷大、操作复杂、自动化程度底、调配费低廉等问题,需已运行或拟新建 PIVAS 医院重点关注。 |

| Objective: To investigate the operation status of pharmacy intravenous admixture service (PIVAS) in children’s (maternal and child) hospitals in China, so as to provide reference for further standardizing the PIVAS work model. Methods: Questionnaire survey was performed with 16 PIVAS managers from children’ s (maternal and child) hospitals in China, and the survey results were statistically analyzed. Results: The PIVAS facilities in the 16 hospitals had areas ranging from 80 to 800 m2 , with an average daily compounding volume per person ranging from 17 to 200 sets. The personnel composition of 12 PIVAS followed the pharmacy-nursing combination model, while 4 followed the single pharmacist model. Intravenous medical orders were reviewed in all 16 PIVAS, 4 of which were reviewed by clinical pharmacists. PIVAS operations covered a wide range of departments and types of drugs, with prescription types including intravenous infusion, intravenous push, and patient-controlled analgesia pumps. Only 4 PIVAS were equipped with automated drug dispensing robots, which were relatively low level of automation. In 16 PIVAS, there were some common issues of drug dosage adjustment, drug combination and drug surplus. The compounding fees for PIVAS varied significantly across regions, with standard fees per set ranging from 15. 00 to 90. 00 yuan for parenteral nutrition solution, 8. 00 to 40. 00 yuan for cytotoxic drugs, 0 to 10. 00 yuan for antibiotics, and 0 to 9. 00 yuan for other common drugs. Conclusion: PIVAS work model in children’s (maternal and child) hospitals has its own characteristics, which also faces challenges such as low numbers of PIVAS, shortage of professional personnel, heavy workloads, complexity of operations, low automation levels and low compounding fees. Hospitals with existing or planned PIVAS facilities should pay special attention to addressing these issues |
