某三甲医院儿科吸入用乙酰半胱氨酸溶液使用情况分析

摘要:
目的:调查某三甲医院儿科吸入用乙酰半胱氨酸溶液临床应用情况,评估住院医嘱合理性,促进儿科吸入用乙酰半胱氨 酸的合理应用,保障儿童安全用药。 方法:回顾性分析某三甲医院儿科 2021 年应用吸入用乙酰半胱氨酸住院患儿临床资料,按 年龄分为<2 岁组和≥2 岁组。 结果:两组患儿性别、体质量、吸入用乙酰半胱氨酸日剂量、雾化单次给药药液量比较差异有统 计学意义,吸入用乙酰半胱氨酸给药频次和用药时间比较差异无统计学意义。 ≥2 岁组药物利用指数高于<2 岁组。 吸入用乙 酰半胱氨酸溶液的住院医嘱存在联合用药不合理、药物配伍不合理、雾化单次给药药液量过高或过低等问题。 结论:吸入用乙 酰半胱氨酸的日剂量、药物利用指数、雾化单次给药药液量随着患儿年龄增长而增大;该院吸入用乙酰半胱氨酸住院医嘱存在 不合理情况,亟待规范。

Abstract:
Objective: To investigate the clinical application of inhaled acetylcysteine solution in pediatrics of a Grade Ⅲ, Level A Hospital, and assess the rationality of inpatient medical orders, so as to promote the rational application of inhaled acetylcysteine in pediatrics and ensure the safe medication of children. Methods: Retrospective analysis was performed on clinical data of pediatric inpatients received inhaled acetylcysteine solution in a Grade Ⅲ, Level A Hospital in 2021. Children were divided into two groups based on age: <2 years old group and ⩾2 years old group. Results: There were statistically significant differences between two groups in terms of gender, body weight, daily dose of inhaled acetylcysteine, and volume of medication per nebulization. However, there were no statistically significant differences in the administration frequency of inhaled acetylcysteine and duration of medication. The drug utilization index in the ⩾2 years old group was higher than that in the < 2 years old group. Inpatient medical orders for inhaled acetylcysteine solution had problems such as inappropriate drug combination, incompatible drug compatibility, and excessive or insufficient medication volume per nebulization. Conclusion: Daily dose of inhaled acetylcysteine, drug utilization index, and medication volume per nebulization increase with the age of children. Medical orders for inhaled acetylcysteine in the hospital are partially irrational and need to be standardized
