| 目的:探讨儿童湿疹的有效治疗方案。 方法:回顾性分析 2022 年 6 月至 2023 年 2 月内蒙古包钢医院收治的 158 例湿疹
患儿的临床资料,根据治疗方法分为观察组 86 例和对照组 72 例,观察组使用西替利嗪口服液联合肤痒清凝胶治疗,对照组采
用西替利嗪口服液治疗,疗程为 28 d,每 7 d 随访 1 次。 比较治疗前后不同时间点患儿湿疹面积及严重程度指数(EASI)评分、
临床症状评分、皮损程度评分、免疫因子水平、炎症因子水平等。 建立广义估计方程模型(GEE),比较治疗效果在不同时间点、
不同组别之间的差异。 结果:两组患儿性别、年龄、病程、胃肠积热、饮食结构、患病史、过敏史、居住环境情况、治疗前 EASI 评
分、临床症状评分及皮损程度评分比较差异无统计学意义(P>0. 05)。 治疗后各时间点患儿 EASI 评分、临床症状评分及皮损程
度评分较治疗前均下降,且观察组改善情况优于对照组(P<0. 05)。 两组患儿 CD4

素(IL)-10 和 IL-17 水平降低且观察组降低更显著(P<0. 01)。 观察组治疗总有效率为 91. 18%,高于对照组的 80. 33%,差异有

| Abstract: |
| Objective: To probe into the effective treatment of children with eczema. Methods: Retrospective analysis was performed
on clinical data of 158 children with eczema admitted into the Inner Mongolia Baogang Hospital from Jun. 2022 to Feb. 2023. According
to different treatment method, all patients were divided into the observation group (86 cases) and the control group (72 cases). The
observation group was treated with cetirizine oral liquid combined with Fuyangqing gel, while the control group received cetirizine oral
liquid. The treatment course was 28 d, and follow-up was performed every 7 d. Eczema area and severity index (EASI) score, clinical
symptom score, skin lesion degree score, immune factor level and inflammatory factor level were compared before and after treatment at
different time points. The generalized estimating equation model (GEE) was established to compare the difference of treatment effect at
different time points and among different groups. Results: There was no significant difference between two groups in gender, age, course
of disease, gastrointestinal fever, diet structure, disease history, allergy history, living environment, EASI score before treatment,
clinical symptom score and skin lesion degree score (P>0. 05). Compared with before treatment, EASI score, clinical symptom score
and lesion degree score decreased after treatment, and the improvement in observation group was better than that in control group (P<
0. 05). The level of CD4

T lymphocytes increased significantly, the level of CD8
+
T lymphocytes, interleukin (IL)-10
and IL-17 decreased significantly, and the changes in the observation group were more significant (P<0. 01). The total effective rate of
observation group was 91. 18%, higher than 80. 33% of control group, with statistically significant difference (P<0. 05). Conclusion:
The efficacy of cetirizine oral liquid combined with Fuyangqing gel in the treatment of children with eczema is significant, which is worthy
of clinical promotion. |
