某院住院患儿静脉用抗菌药物应用情况及非整支注射药品精准调配

摘要:
目的:回顾性分析某院住院患儿静脉用抗菌药物使用情况,并探讨非整支注射药品的精准调配方法,为患儿抗菌药物合理使用提供参考。 方法:选取 2021 年我院静脉用药集中调配中心(PIVAS)的 378 例住院患儿,对患儿基本情况、抗菌药物使用情况及联合用药进行统计分析,并制定非整支注射药品的精准调配规范。 结果:378 例住院患儿中,男女比例 1. 30 ∶ 1,平均年龄 4. 02 岁,平均住院时间 7. 97 d;疾病类型以肺炎(62. 70%)为主,其次为胃肠炎(11. 11%)、急性上呼吸道感染(3. 97%);涉及16 种静脉用抗菌药物,其中注射用粉针剂 15 种,注射液 1 种,人均使用 1. 37 种,使用率排名前三的药物是头孢他啶(45. 77%)、头孢唑肟钠(24. 34%)、注射用阿奇霉素(23. 02%);74 例存在联合用药情况,其中 2 种抗菌药物联用 73 例,3 种抗菌药物联用1 例;108 例患儿在住院期间更换过抗菌药物,换药率为 28. 57%;制定了非整支注射药品调配规范,保证静脉输液的剂量精准。结论:我院儿童静脉用抗菌药物存在不合理之处,应加强抗菌药物管理,并提高对非整支药品调配的精准性,为患儿提供优质的药学服务。

Application of Intravenous Antibiotics and Exploration on Precise Dispensing of Non-Integral Injections inHospitalized Children in a Hospital

Abstract:
Objective: To retrospectively analyze the application of intravenous antibiotics in hospitalized children, and to explore theprecise dispensing method of non-integral injections, so as to provide reference for rational application of antibiotics in children.Methods: Totally 378 hospitalized children were extracted in the Pharmacy Intravenous Admixture Services (PIVAS) of our hospital in2021. General information, application of antibiotics and drug combination were statistically analyzed. Relevant regulations of nonintegral injections were formulated. Results: Among the 378 hospitalized children, the ratio of male to female was 1. 30 ∶ 1, and theaverage age and length of stay were respectively 4. 02 years and 7. 97 d. The main disease types were pneumonia (62. 70%), followedby gastroenteritis (11. 11%) and acute upper respiratory tract infection (3. 97%). A total of 16 kinds of drugs were involved, of which15 were powder injection and 1 was injection solution, with the per capita use of 1. 37 categories, the top three drugs ranked byapplication rate were ceftazidime (45. 77%), ceftizoxime sodium (24. 34%) and azithromycin (23. 02%). A total of 74 cases had drugcombination, including 73 cases of two kinds of antibiotics and 1 case of three kinds of antibiotics. There were 108 children who changedantibiotics during hospitalization, with the ratio of 28. 57%. Relevant regulations on dispensing of non-integral injections had beenformulated to ensure the accurate dosage of intravenous infusion. Conclusion: In response to the irrationality of intravenous antibioticsfor hospitalized children in our hospital, the management of antibiotics should be strengthened and the precise dispensing of non-integralinjections should be improved to provide better pharmaceutical care for children.
