| 目的:总结硝酸甘油在新生儿外周组织缺血中的应用。 方法:回顾性分析 2022 年 3 月我院 1 例硝酸甘油治疗早产儿外周组织缺血的临床资料,检索中国知网、万方和 PubMed 数据库筛选使用硝酸甘油治疗新生儿外周组织缺血患儿的临床资料,采用描述性统计分析方法。 结果:本研究 1 例早产儿经外周置入中心静脉导管(PICC)置管 1 h 后,穿刺肢体发生外周组织缺血,常规处理 8 h 无改善后使用硝酸甘油成功治愈。 本研究(1 例)及文献报道(47 例)共 48 例新生儿发生外周组织缺血首次使用硝酸甘油的中位时间为外周组织缺血发生后 3(0,192)h,中位治疗时间 2(0. 02,32)d。 79. 2%(38 / 48)患儿完全康复,20. 8%(10 / 48)发生组织坏死;95. 8%(46 / 48)顺利出院,4. 2%(2 / 48)在后续治疗中死亡。 结论:硝酸甘油在新生儿外周组织缺血治疗中有良好的疗效和安全性,早期应用效果显著。 |

| Objective: To summarize the application of nitroglycerin in neonatal peripheral tissue ischemia. Methods: Clinical data ofa preterm infant with peripheral tissue ischemia treated with nitroglycerin in our hospital in Mar. 2022 were retrospectively analyzed.CNKI, Wanfang and PubMed databases were retrieved to collect application data of nitroglycerin in neonates with peripheral tissueischemia. Descriptive statistical analysis method was adopted. Results: Peripheral tissue ischemia occurred in the punctured limb afterperipheral central venous catheter (PICC) insertion for 1 h, and was successfully cured by nitroglycerin after routine treatment for 8 hwithout improvement. The median time of first administration of nitroglycerin in 48 neonates ( 1 case in this study and 47 cases inreported literature) with peripheral tissue ischemia was 3 ( 0, 192) h after peripheral tissue ischemia and 2 ( 0. 02, 32) d aftertreatment. Totally 79. 2% ( 38 / 48) recovered completely, 20. 8% ( 10 / 48) had tissue necrosis, 95. 8% ( 46 / 48) were dischargedsuccessfully, and 4. 2% (2 / 48) died during follow-up treatment. Conclusion: Nitroglycerin has significant efficacy and safety profile inthe treatment of peripheral tissue ischemia in neonates with early application. |
