1 例拉考沙胺致儿童药物性肝损伤病例分析并文献复习

摘要:
目的:探讨拉考沙胺(LCM)导致药物性肝损伤(DILI)的相关性和特点,为 LCM 的安全使用提供参考。 方法:分析某院 1 例 9 岁儿童因治疗癫痫使用注射用 LCM 后导致 DILI 的案例,并复习国内外相关文献。 结果:检索到国内外 LCM 致 DILI 相关 文献共 5 篇,而本文为首例儿童使用 LCM 导致 DILI 的报道,分析发现在使用 LCM 治疗期间有可能会出现不同程度的 DILI,主 要为肝细胞损伤型,在停药或对症治疗后肝功能均逐步恢复正常。 结论:LCM 致 DILI 鲜有报道,肝损伤的病理机制尚不清楚, 临床应考虑 LCM 致 DILI 的可能性,服药期间关注肝功能变化情况。

A Case of Drug-Induced Liver Injury Induced by Lacosamide in Children and Literature Review

Abstract:
Objective: To probe into the correlation and characteristics of drug-induced liver injury (DILI) induced by lacosamide (LCM), so as to provide reference for the safe use of LCM. Methods: A case of a 9-year-old child in a certain hospital who developed DILI induced by LCM for epilepsy treatment was analyzed. Relevant literature at home and abroad were reviewed. Results: A total of 5 domestic and foreign literature related to DILI induced by LCM were retrieved. This article was the first report of children with DILI induced by LCM. The analysis showed that DILI of different degrees may occur during the treatment of LCM, mainly liver injury type, and liver function returned to the normal state after drug withdrawal or symptomatic treatment. Conclusion: There are few reports of DILI induced by LCM, and the pathological mechanism of liver injury is not yet clear. In clinical practice, the possibility of DILI induced by LCM should be recognized, and changes in liver function should be monitored during medication.
