亚高原地区儿童房间隔缺损及动脉导管未闭术后生长发育、心脏结 构变化的临床研究

摘要:
目的:探讨儿童先天性心脏病对生长发育的影响以及手术后的追赶效果,为先天性心脏病的规范化筛查及手术治疗时 机选择提供依据。方法:采用前瞻性队列研究方法,选取2022 年海拔2 600 米及以上的丽江市宁蒗县5 个乡镇进行筛查,完成 手术前后先天性心脏病的人口学信息收集,通过自身前后对照的方式,比较手术前后亚高原地区先天性心脏病患儿生长发育的 变化。结果:研究共筛查8 965 人,明确诊断为先天性心脏病136 例,完成手术和1 年随访的共42 例,其中男16 例,女26 例,房 间隔缺损(ASD)28 例,动脉导管未闭(PDA)14 例。手术前后身高、体质量、体质量指数(BMI)变化比较差异有统计学意义。对 比全国同性别、同龄健康儿童或青少年,先天性心脏病患儿整体生长发育指标均偏低。其中,相对年龄越大,落后越明显。小年 龄组(5~10 岁)术后追赶最快,此年龄段男性较女性明显。结论:先天性心脏病在亚高原地区儿童中的发生率依然较高。接受手 术治疗的先天性心脏病儿童,在学龄前及学龄初期生长发育仍有追赶的可能。这一时期亦可视为相对适宜的筛查与干预阶段。

Abstract:
Objective: To probe into the impact of congenital heart disease on growth and development, as well as the catch-up effect after surgery, so as to provide basis for standardized screening and selection of surgical treatment timing for congenital heart disease. Methods: A prospective cohort study was conducted in five towns in Ninglang County, Lijiang City, at an altitude of 2,600 meters, in 2022. The population demographic information of children with congenital heart disease before and after surgery was collected, and the changes in growth and development of children with congenital heart disease in the sub-high plateau area before and after surgery were compared by using the self-controlled case series design. Results: A total of 8,965 cases were screened, and 136 cases of congenital heart disease were confirmed, of which 42 cases underwent surgery and one-year follow-up, including 16 cases of males and 26 cases of females. Among them, 28 cases were atrial septal defect (ASD), and 14 cases were patent ductus arteriosus (PDA). There was statistically significant difference in height, weight, and body mass index (BMI) before and after surgery. Compared with nationally representative age-matched healthy children or adolescents, children with congenital heart disease had overall lower growth and development indicators, and the difference was more obvious as the relative age was older. The catch-up growth after surgery was the fastest in the age group of 5 to 10 years, and the difference was more obvious in males than in females in this age group. Conclusion: Congenital heart disease in children from the sub-high plateau area is still not uncommon. Children with congenital heart disease who undergoing surgery during the preschool and early school-age periods can still catch up in terms of growth and development. This period can also serve as a relatively suitable stage for screening and intervention.
