| 目的:探讨有血流动力学意义的动脉导管未闭( hsPDA) 早产儿单疗程口服布洛芬治疗失败的危险因素。方法:选取
2018-2023 年首都医科大学附属北京妇产医院新生儿科住院,胎龄≤34 周,生后72 小时行床边超声心动图确诊hsPDA 并口服
布洛芬治疗的早产儿为研究对象,治疗后24 小时内复查床边超声心动图,根据PDA 关闭情况分为关闭组和未关闭组,分析单
疗程口服布洛芬治疗后影响hsPDA 关闭的高危因素。结果:单因素分析结果显示,两组患儿中重度肺动脉高压、喂养不耐受、
化脓性脑膜炎发生率比较差异有统计学意义(P 均<0. 05)。多因素logistic 回归分析显示,喂养不耐受是单疗程口服布洛芬治
疗早产儿hsPDA 关闭失败的独立危险因素(P<0. 05)。结论:中重度肺动脉高压、喂养不耐受、化脓性脑膜炎是hsPDA 早产儿
单疗程口服布洛芬治疗失败的高危因素,其中喂养不耐受是导致hsPDA 关闭失败的独立危险因素。 |

| Abstract: |
| Objective: To probe into the the risk factors for failure of single-course oral ibuprofen therapy in preterm infants with
hemodynamically significant patent ductus arteriosus (hsPDA). Methods: Preterm infants (gestational age ≤34 weeks) admitted into
the Department of Neonatology, Beijing Obstetrics and Gynecology Hospital, Capital Medical University from 2018 to 2023, diagnosed
with hsPDA by bedside echocardiography within 72 h after birth and underwent oral ibuprofen therapy were extracted. All infants
underwent bedside echocardiography within 24 h after oral ibuprofen treatment. Based on PDA closure status, the cohort was divided into
the closure group and non-closure group. High-risk factors influencing hsPDA closure after single-course oral ibuprofen therapy were
analyzed. Results: Univariate analysis showed that there were significant differences in moderate and severe pulmonary hypertension,
feeding intolerance and purulent meningitis between two groups (P<0. 05). Multivariate logistic regression analysis showed that feeding
intolerance was an independent risk factor (P<0. 05). Conclusion: Moderate and severe pulmonary hypertension, feeding intolerance,
and purulent meningitis are high-risk factors for failure of hsPDA closure following a single-course oral ibuprofen therapy in preterm
infants. Among these, feeding intolerance is an independent risk factor contributing to failure of hsPDA closure. |
