| 目的:检索利奈唑胺(LZD)致新生儿不良反应的文献,收集相关信息,为临床安全用药提供参考。方法:检索万方、中国
知网、Wiley Onlinelibrary 和PubMed 等数据库(检索时限从建库至2025 年3 月15 日),收集住院期间应用LZD 治疗出现不良反
应的新生儿临床资料,提取患儿基本信息,统计分析其不良反应类型及发生原因。结果:共收集新生儿期应用LZD 出现不良反
应的文献报道15 篇,纳入患儿553 例,其中男317 例(57. 32%),女236 例(42. 68%),胎龄24~42 周,出生体质量350~4 300 g,
首次用药日龄1~47 d,发生不良反应107 例(19. 35%)。81 例患儿表现为血液系统改变,其中63 例表现为不同程度的血小板
减少。21 例患儿出现消化系统症状,3 例出现代谢障碍,2 例出现神经系统病变。停用LZD 并予对症支持治疗后,2 例患儿失
访,104 例患儿相关不良反应均恢复正常,仅1 例听神经病变不可逆。结论:新生儿应用LZD 所致不良反应主要为血小板减少,
停药后可恢复正常;罕见高乳酸血症、听神经损伤。临床应做好全程用药监护,及时完善眼科及听力筛查。 |

| Objective: To retrieve literature on neonatal adverse drug reactions induced by linezolid (LZD) and collect relevant
information, so as to provide reference for safe clinical medication. Methods: Wanfang, CNKI, Wiley Onlinelibrary and PubMed
databases (the retrieval time was up to Mar. 15, 2025) were retrieved to collect clinical data of neonates with adverse drug reactions
induced by LZD during hospitalization. The basic information was extracted, and the types and causes of adverse drug reactions were
statistically analyzed. Results: A total of 15 literature on adverse drug reactions induced by LZD in the neonates were collected,
including 553 children, with 317 males (57. 32%) and 236 females (42. 68%). Gestational ages ranged from 24 to 42 weeks, with
birth weights ranging from 350 to 4,300 g and postnatal ages ranging from 1 to 47 d. Adverse drug reactions were observed in 107 cases
(19. 35%). Hematological changes were the most common, reported in 81 cases, with thrombocytopenia of varying severity observed in
63 cases. Gastrointestinal symptoms were reported in 21 cases, metabolic disorders in 3 cases, and neurological lesions in 2 cases.
Following the discontinuation of LZD and symptomatic supportive treatment, 2 cases with loss to follow-up, adverse drug reactions were
resolved in 104 cases, except for one case of irreversible auditory neuropathy. Conclusion: The main adverse drug reaction of LZD in
neonates was thrombocytopenia, which could be return to normal state after withdrawal. Hyperlactatemia and auditory nerve injury are
rare. Continuous monitoring during treatment and timely ophthalmologic and hearing assessments are recommended to ensure medication safety. |
