| 目的:分析1 例长期使用利奈唑胺诱发纯红细胞再生障碍性贫血和周围神经病变的临床病例,提升临床使用利奈唑胺过程
中对该药药物不良反应的监测和识别能力。方法:回顾性分析上海市儿童医院收治的1 例利奈唑胺致纯红细胞再生障碍性贫血
和双下肢神经病变的病例,并回顾相关文献。结果:患儿因右胫骨骨髓炎,使用利奈唑胺片(每次600 mg,bid)3 个月后出现重度贫
血和双下肢感觉异常,诊断利奈唑胺致纯红细胞再生障碍性贫血和双下肢神经病变,停利奈唑胺2 周后,患儿贫血和双下肢感觉
异常症状好转出院。结论:利奈唑胺的剂量、用药持续时间、用药前血红蛋白水平、肝肾功能不全、用药前血清白蛋白水平、联合用
药是导致相关贫血和周围神经病变的危险因素,关注该药不良反应的危险因素,监测药物浓度有利于早期发现,早期干预。 |

| Linezolid-Induced Pure Red Cell Aplasia Complicated with Peripheral Neuropathy in Lower Limbs: a CaseReport and Literature Review |

| Abstract: |
| Objective: To analyze a clinical case of pure red cell aplasia complicated with peripheral neuropathy induced by long-term
application of linezolid, and to enhance the ability of monitoring and identifying the adverse drug reactions of linezolid in the clinic.
Methods: Retrospective analysis was performed on a case of pure red cell aplasia complicated with peripheral neuropathy in lower limbs
induced by linezolid in Shanghai Children’ s Hospital, and relevant literature was reviewed. Results: The child, with right tibial
osteomyelitis developed severe anemia and peripheral neuropathy in lower limbs after 3 months of linezolid therapy (600 mg, bid), was
diagnosed with pure red cell aplasia complicated with peripheral neuropathy in lower limbs induced by linezolid. After 2 weeks of
linezolid withdrawal, the child was discharged with improved symptoms of anemia complicated with peripheral neuropathy in lower limbs.
Conclusion: The dosage of linezolid, duration of medication, hemoglobin level before administration, renal insufficiency and liver
dysfunction, serum albumin level before administration, and drug combination are risk factors for linezolid-induced anemia and
peripheral neuropathy. Paying attention to the risk factors of adverse drug reactions and monitoring drug concentration are conducive to
timely detection and early intervention. |
