| 目的:通过信息化管理与PDCA 循环管理相结合的持续质量改进方式,降低儿童专科医院静脉用药调配中心(PIVAS)的
调剂差错率。方法:通过成立质量改进小组,构建信息化差错登记系统,采用个人数字助手(PDA)扫码登记和内部差错(内差)
登记激励机制,提高数据收集效率与完整性。对审方、调制、配制、成品核对等环节的差错进行全过程监控与分类管理,并实施
PDCA 循环进行持续质量改进。结果:持续质量改进后,我院PIVAS 总体差错率呈逐渐下降趋势,其中差错持续2 环节、差错持
续3 环节和配制环节的差错率分别下降59. 0%、43. 8%、37. 5%,药品损失率降低50. 0%,工作质量明显提升。结论:信息化差错
登记与PDCA 循环的协同应用,可有效降低PIVAS 的调剂差错率,提升儿童用药安全性,为医疗机构质量改进提供参考。 |

| Objective: To reduce the dispensing errors in the Pharmacy Intravenous Admixture Service (PIVAS) of a children’ s
hospital through PDCA continuous quality improvement combined with information-based management. Methods: A quality improvement
team was established to develop an information-based error reporting system. The incentive mechanism of personal digital assistant
(PDA) scan code registration and differential registration was adopted to improve the efficiency and integrity of data collection. Errors
were monitored and categorized across all workflow stages, including prescription review, dispensing, admixture, and final verification.
The PDCA cycle was applied to evaluate the impact on quality improvement. Results: After continuous quality improvement, the overall
error rate showed a gradual decline. Specifically, the error rates in the two-stage continuous error, three-stage continuous error and
admixture error decreased by 59. 0%, 43. 8%, and 37. 5%, respectively. Additionally, the medication loss rate was reduced by
50. 0%, demonstrating significant improvement in work quality. Conclusion: The synergistic application of information-based error
reporting system and PDCA cycle could effectively reduce PIVAS dispensing errors and enhance medication safety for pediatric patients,
which provides reference for quality improvement in healthcare institutions. |
