| Abstract: |
| Objective: To evaluate the thyroid function of extremely premature infants and explore the significance of using levothyroxine
sodium for replacement therapy. Methods: Clinical data of extremely preterm infants admitted into Beijing Obstetrics and Gynecology
Hospital, Capital Medical University from Jul. 2021 to Jul. 2024 were retrospectively analyzed to summarize the thyroid function,
clinical features and outcomes. Results: Among 38 enrolled infants, 10 cases had thyroid dysfunction and 9 cases received levothyroxine
sodium replacement. Patients were stratified into case (dysfunction) group and control (normal thyroid function) group. Clinical data
and outcomes of two groups were analyzed. The case group had prolonged jaundice and delayed full enteral feeding (P=0.014 and 0.027). No
intergroup differences were found in bronchopulmonary dysplasia, intraventricular hemorrhage, surgical retinopathy of prematurity, pass
rate of hearing screening or neonatal behavioral neurological assessment scores ( P > 0. 05). Conclusion: Despite levothyroxine
replacement therapy is administered to most extremely preterm infants with thyroid dysfunction, prolonged jaundice resolution and
delayed feeding are still observed. After replacement therapy, no statistically significant differences are found in short-term respiratory
and neurological outcomes during hospitalization ( intracranial hemorrhage, surgically treated retinopathy of prematurity, neonatal
behavioral neurological assessment score, and pass rate of hearing screening) compared with infants having normal thyroid function. |
