泉州地区5岁以下住院患儿呼吸道感染非细菌病原体检测结果分析

摘要:
目的:检测儿内科5岁以下呼吸道感染住院患儿的非细菌病原体IgM抗体,为临床诊疗提供依据。方法:收集11 976例5岁以下儿内科呼吸道感染住院患儿的血清标本,采用间接免疫荧光法检测其中8种常见非细菌病原体的IgM抗体。结果:检测11 976例标本非细菌病原体IgM抗体,其中流感病毒B型(FluB)、肺炎支原体(MP)阳性率较高,其次为呼吸道合胞病毒(RSV)、流感病毒A型(FluA)、嗜肺军团菌(LP)。MP在2013-2014 年5、6、7、8月份的阳性率均较高,FluA在1、2月份和11、12月份感染率高,FluB、副流感病毒在6、12月份的感染率高于其他月份。MP阳性率在春夏季显著高于秋冬季。MP感染率2岁以上儿童高于2岁以下婴幼儿,RSV主要感染2岁以下的婴幼儿。结论: 2012年11月至2015年8月泉州地区引起5岁以下儿童呼吸道感染的非细菌病原体主要是FluB、MP,不同病原体在不同的月份感染率有差别。MP在春夏季的感染率较高。2岁以上儿童MP感染率较高,而RSV主要感染2岁以下的婴幼儿。

Abstract:
Objective: To investigate the IgM antibodies against non-bacterial pathogens of hospitalized children less than 5 years in Quanzhou area, and to provide evidences for clinical diagnosis and treatment. Methods: A total of 11,976 blood serum samples from hospitalized children less than 5 years old with respiratory tract infection were collected. Indirect immunofluorescence assay was applied to detect IgM antibodies of 8 common pathogens. Results: IgM antibodies of non-bacterial pathogens were detected in 11,976 samples. Influenza virus types B (FluB) and Mycoplasma pneumoniae (MP) showed the higher positive rates, followed by Respiratory syncytial virus (RSV), Influenza virus types A (FluA), Legionella pneumophila (LP). The positive rate of MP was relatively high in May, June, July and August, and that of FluA was the highest in January, February, November and December. FluB and Parainfluenza virus (PIV) were found to be popular in June and December. The positive rate of MP showed statistically significant higher in spring and summer than those in autumn and winter. MP infection was more common in children older than 2 years old, while RSV infection was more common in infants less than 2 year. Conclusion: FluB and MP are the main non-bacteria pathogens for respiratory tract infection of children aged less than 5 years old in Quanzhou area in recent 3 years. The positive rate of each pathogen is variable in difference mouths. The positive rate of MP is relatively higher in spring and summer. MP infection is more common in children older than 2 years. RSV infection is more common in infants less than 2 year.
