| 目的:统计分析我院住院不合理用药医嘱,为临床合理用药提供参考。方法:对我院2015年1-12月登记的不合理医嘱进行统计分析。结果:共有1 633例不合理医嘱。不合理医嘱主要涉及药物剂量不当(37.84%)、出院带药无用法用量(16.66%)、给药方式错误(10.90%)、药物浓度不当(8.14%)、输液速度不当或无(8.08%) 等。结论:我院住院医嘱存在一定的不合理现象,药师通过前瞻性实时审方并进行干预,可防止差错事件发生,保障临床用药安全。 |

| Objective: To analyze statistically the irrational medical advices in our hospital, and provide reference for clinical rational drug use. Methods: The irrational medical advices in our hospital were summarized and analyzed statistically from January to December in 2015. Results: A total of 1,633 irrational medical advices were reviewed. Irrational medical advices mainly contain improper drug dose (37.84%), discharge medication without usage and dosage (16.66%), wrong administration approach (10.90%), wrong drug concentration (8.14%), improper infusion speed (8.08%), etc. Conclusion: Some advices are unreasonable in our hospital, the pharmacist intervention by auditing medical orders in real time can prevent medical errors, ensure the drug safety of patients. |
