咪达唑仑经鼻喷雾用于幼儿术前焦虑的疗效评价

摘要:
目的:评价咪达唑仑经鼻喷雾用于幼儿术前焦虑的临床疗效。方法:80 例拟在全麻下行择期手术的幼儿( 年龄1~3岁),美国麻醉医师协会(ASA)生理状态评分I~II 域级,随机分为咪达唑仑组和生理盐水组各40例,进入手术室前5 min采用MAD NasalTM经鼻黏膜喷雾给药装置分别给予咪达唑仑(5 mg/mL)和生理盐水0.04 mL/kg。观察并记录患儿与家长分离时的镇静评分(G评分)、麻醉诱导时的面罩接受程度评分(S评分)、术毕拔管时间、麻醉苏醒期躁动评分(EA评分)及不良反应。结果:咪达唑仑组G评分、S评分均高于生理盐水组(P<0.01),EA评分低于生理盐水组(P<0.01)。两组术毕拔管时间比较差 异无统计学意义(P>0.05)。两组均未见术前用药或麻醉相关不良反应。结论:咪达唑仑经鼻喷雾可有效用于幼儿术前焦虑。

The Efficacy of Midazolam Nasal Spray for Anti鄄Anxiety Premedication in Toddlers

Abstract:
Objective: To assess the efficacy of midazolam nasal spray for anti-anxiety premedication in toddlers. Methods: Eighty children, aged 1 to 3 years, scheduled to selective surgery, with American Society of Anesthesiologists (ASA) physical states score I to II grade. All children were randomly assigned to two groups: midazolam group and control group. Five minutes before entering the operation room, nasal spray of midazolam (5 mg/mL) or normal saline (0.04 mL/kg) was given to midazolam group and control group respectively. Sedation score at separation with parents, face mask acceptance score at anesthesia induction, extubation time after surgery, emergence agitation score at the recovery period, and any of adverse reactions related to premedication or anesthesia, were recorded. Results: The sedation scores at separation with parents and the face mask acceptance scores at anesthesia induction in midazolam group, were significant higher than those in control group (P<0.01). The emergence agitation scores in midazolam group were significant lower than that in control group (P<0.01). The extubation time was similar between midazolam group and control group (P>0.05), and no premedication or anesthesia related adverse reactions was observed. Conclusion:Midazolam nasal spray can be used effectively as anti-anxiety premedication in toddlers.
