布地奈德联合沙丁胺醇雾化吸入治疗小儿毛细支气管炎的效果及其对患儿免疫功能的影响

摘要:
目的:探讨雾化吸入布地奈德混悬液和沙丁胺醇治疗小儿毛细支气管炎的临床疗效,并观察患儿血清IgE、T细胞亚群水平的变化。方法:选取2012年4月至2016年4月在我院儿科接受治疗的毛细支气管炎患儿76例,随机分为观察组和对照组各38例。观察组给予布地奈德混悬液联合沙丁胺醇雾化吸入治疗,对照组仅给予布地奈德混悬液雾化吸入治疗,观察记录临床疗效、患儿治疗前后血清IgE及T细胞亚群水平。结果:观察组痊愈率(73.68%)和总有效率(94.74%)均高于对照组(P均<0.01)。两组患儿治疗后CD4+、CD4+/CD8+水平均较治疗前升高,血清IgE、CD8+水平均较治疗前降低(P均<0.05)。观察组患儿治疗后血清IgE、CD8+水平均低于对照组,CD4+、CD4+/CD8+水平均高于对照组(P均<0.05)。结论:布地奈德联合沙丁胺醇雾化吸入治疗小儿毛细支气管炎具有协同作用,能有效缓解毛细支气管炎患儿的临床症状,调节患儿机体免疫功能,减轻炎性反应,有利于患儿的康复。

Abstract:
Objective: To evaluate clinical effect of aerosol inhalation of budesonide suspension combined with albuterol in treatment of capillary bronchiolitis, and to observe the changes of IgE and T cell subsets. Methods: Seventy six capillary bronchiolitis children were randomly divided into observation group (38 cases) and control group (38 cases). Observation group was given aerosol inhalation of budesonide suspension combined with albuterol, the control group were treated only by aerosol inhalation of budesonide suspension. The clinical efficacy, the levels of IgE and T cell subsets before and after treatment were observed and recorded. Results: The cure rate and total effective rate of the observation group were higher than those of the control group. The levels of CD4+ and CD4+/CD8+ of two groups were higher than those before treatment, the levels of IgE and CD8+ of two groups were lower than those before treatment. And the levels of IgE and CD8+ of observation group were lower than those of control group after treatment, the levels of CD4+ and CD4+/CD8+ of observation group were higher than those of control group after treatment. The differences between two groups were statistically significant (P<0.05). Conclusion: Aerosol inhalation of budesonide suspension combined with albuterol for capillary bronchitis which has a synergy, can relieve clinical symptoms effectively, regulate their immune function, reduce the inflammatory reaction and be beneficial to the recovery of children.
