儿童泌尿道感染980株病原菌分布及耐药性分析

摘要:
目的:探讨2013-2015年昆明市儿童医院泌尿道感染患儿病原菌构成和耐药特点,以期为临床经验用药提供参考依据。方法:收集我院2013-2015年13 929例门诊和住院泌尿道感染患儿的清洁中段尿,对尿液进行细菌培养鉴定及细菌耐药性分析。结果:13 929例尿样中菌检阳性率为7.8%,在排除重复菌株后共检出980株病原菌,其中革兰阴性杆菌668株(68.2%),革兰阳性球菌308株(31.4%),真菌4株(0.4%)。居前两位的菌种为大肠埃希菌、肠球菌,二者所占比例总和达到71.2%。药敏试验结果显示,两种主要致病菌对多种抗生素耐药性高。结论:大肠埃希菌和肠球菌是儿童泌尿道感染的主要致病菌,其分布在我院2013-201 年无明显变化,但均表现出较强的多重耐药性。明确泌尿道感染的病原菌分布及这些病原菌对抗生素的耐药性,对儿童尿路感染的治疗及抗生素合理使用都具有重要意义。

Pathogen Distribution and Drug Resistance Analysis on 980 Strains of Children Urinary Tract Infection

Abstract:
Objective: To investigate the pathogen distribution and drug resistance in children with urinary tract infection (UTI) in Kunming Children爷s Hospital in 2013 to 2015, in order to provide the basis for clinical experience. Methods: Collected and analyzed the 13,929 cases of midstream urine samples of children from 2013 to 2015, identification of bacterial culture and analysis of drug resistance were taken. Results: The positive rate was 7.8% in 13,929 cases of urine, 980 strains of pathogenic bacteria were detected by exclusion of the repetitive strains, including 668 strains (68.2%) of gram-negative bacteria, 308 strains (31.4%) of gram-positive bacteria, and 4 strains (0.4%) of fungi. The top two were Escherichia coli, Enterococcus, the proportion of the sum of the two reached to 71.2%, the antimicrobial susceptibility testing showed the two main types of pathogens had high resistance to many antibiotics. Conclusion: Escherichia coli and Enterococcus are the main pathogens of urinary tract infection in children, and the distributions of the isolated pathogens were not significantly changed in 2013 to 2015. Furthermore, Escherichia coli and Enterococcus showed strong drug multi-resistance. Pathogens distribution and antibiotics resistance are very important for diagnosis and treatment the urinary tract infection in children.
