| 目的:探讨血管紧张素转化酶抑制剂(ACEI)在小儿肾脏疾病中的应用,为规范该类药物在IgA肾病中的超说明书应用提供参考依据。方法:回顾性分析2014年12月16日1例7岁IgA肾病患儿的病例资料,结合最新的临床证据,从超说明书用药角度分析和评价贝那普利在IgA肾病儿童患儿中的应用,探讨并明确ACEI类药物在慢性肾病患者中的应用指征、剂量及疗程等超说明书用药内容。结果:贝那普利具有不依赖于降血压的降蛋白作用,在应用ACEI类药物时需要监测肾功能以评估病情变化并调整药物剂量。结论:ACEI类药物在儿童肾脏病中有积极的治疗作用,应严格把握该类药物的超说明书用药指征,加强临床观察和药学监护,确保用药安全。 |

| Objective: To discuss the application of angiotension converting enzyme inhibitors(ACEI) in children, in order to provide references for clinical off-lable drug use of ACEI in IgA nephropathy. Methods: Combined with the latest clinical evidence and one case of IgA nephropathy in a 7-year-old boy, the off-label drug use of benazepril in IgA nephropathy was analyzed and evaluated. Meanwhile, the indication of medication, dosage and course of treatment were discussed and explicated. Results: The renal protection conferred by benazepril was not dependent on its blood pressure, and the drug need to be adjusted by monitoring renal functions. Conclusion: Physicians should be affirmed to this potential role of ACEI in the treatment of kidney disease. To protect the safety of children, it requires strictly managing the off-label drug use of ACEI, strengthening clinical observation and pharmaceutical care. |
